Contact Allergies

The symptoms of contact allergies normally occur within a few moments of exposure to the allergen; however, it is possible for reactions to the allergens to show up several hours after exposure. There are many common allergens such as poison oak and poison ivy as well as medications, latex, dyes, fragrances or perfumes and cosmetics, preservatives and nickel, rubber and hair dyes. Nickel is a frequently used metal in jewelry, hairpins, buttons and zippers as well as snaps and metal clips and is frequently associated with allergies. How to Burn More Calories Without Working Out Longer

Time is a limiting factor for many people who workout and want to workout. Even the most good intentioned dedicated gym goers and workout enthusiasts sometimes find themselves in a bind with other commitments and cannot “find the time” to get to the gym. The ever increasing pace of life requires us to be very good at time management to fit everything we want into a day, especially our workouts. When it comes to working out I’ll bet you have a set amount of time you can dedicate to it each week and that’s it, there is no room for increasing that time. This is probably the case for almost everyone that works out on a regular basis. So if we can’t ever increase the time we spend working out, how to do we increase the results we are getting? There are a few things you can do.

In general there are only 2 basic things you can change in each workout to progress and continue to get the benefits you want.

1) Increase the amount of time working out (but as we are discussing here this is not an option for most people)

2) Increase the intensity of each workout. This can be accomplished multiple ways:
a. Increasing the amount of weight you are using
b. Use more muscles with each exercise
c. Do more reps in the same amount of time
d. Decrease the amount of rest you take between exercises

Increasing Weight used:

Even if you aren’t doing traditional weight training, you can increase the amount of work you are doing by adding some weight to any exercise you are doing. For example of you do some interval running and want to increase the amount of work you are doing you can add a weighted vest for some of your run. A vest that adds 5 or 10lbs to your weight will make you work much harder during your run. You will end up burning more calories and improve your conditioning beyond just running with your regular body weight. You wouldn’t have to do your whole run with the vest; maybe just add in 20% of the run with the vest to start.

If you are doing weights in your workout (which I would highly recommend as a regular part of your weekly workout routine) then the easiest way to increase the amount of work you are doing is to increase the weight. This means increasing the weight and doing the same number of reps. This technique works if you are working with weights well below the maximum weight you can handle. If you are already working at or near your maximum weights then you will obviously not be able to increase the weight as by definition of max you would not be able to lift anything heavier. This however is not the case for most people, and there is usually some room to move up. Even if you only go up 5lbs, that is still more work done.

Use More Muscles with Each Exercise

This is a simple and very effective way to get more work done without extending the time. For example if you are doing a bodyweight squat, you can add in a shoulder press at the same time. Now you are working your legs and hips as well as your shoulders and arms. This is substantially more work than squats by themselves. This technique of using more muscles can be applied to almost any exercise you can think of.

More Reps in the same amount of Time

This simply means doing your reps faster. So if you can do 10 bicep curls in 10 seconds. Try to do 15 in 10 seconds (with the same weight). This will be 50% more work done. You can apply this technique to almost any exercise. In the case of running or other cardio type exercises, this would just be taking more steps, or in other words, running faster. I would suggest adding in short bursts of faster running, and more reps. For example if you go for a 30 minute run. Now you could add in 30 second bursts of running faster every 5 minutes. This would increase the work done without increasing your time spent.

Decrease the Rest between Exercises

If you do an intermittent style of workout as many people do, there will be periods of rest between exercise sets. Shorten your rest periods and the amount of work you are doing will increase. This also allows you to fit a few more sets in the same amount of time.

You can apply any of these techniques to increase the amount of work you are doing and the amount of calories you are burning without every working out for any longer. I have designed workouts that incorporate most of these techniques and they produce great results. This is the new are of working out with incredible time constraints so the workout you are doing must be adaptable if you want to continue to progress.

Just How Much Vitamin C Do You Need For Optimum Health?

Many benefits of vitamin C have been identified since the consumption of citrus fruit was first recognised as the immediately effective cure and preventative for the scurvy which so dreadfully afflicted the long distance sailors of a few centuries ago.

Numerous studies have recognised the vitamin as a possible protector against coronary heart disease, stroke, atherosclerosis, hypertension and cancer, to name but a few, and it is possible that all of these effects stem ultimately from vitamin C's acknowledged role as a powerful anti-oxidant, active in preventing the free radical damage which is a known cause of premature ageing and many chronic degenerative diseases.

So the absence of acute deficiency diseases such as scurvy in modern affluent societies should not be taken as implying that the typical Western diet contains an adequate intake of vitamin C for optimum long-term health.

The United States Recommended Dietary Allowance (RDA) for vitamin C is 90 mg per day for a healthy, non-smoking man and 75 mg for a woman. But these suggested requirements are increased to 125mg and 110 mg respectively in the case of smokers, a tacit acknowledgement of the increased requirement for this vitamin which increased toxic stress places on the body. But these RDAs nevertheless appear to be set at a level sufficient only to avoid outright deficiency disease.

To be sure of obtaining all the optimum benefits of vitamin C, the authoritative Linus Pauling Institute has recommended a daily minimum intake of at least 400 mg. This amount has been shown to achieve saturation levels of the vitamin within the body's tissues in healthy young adults who are non-smokers. To get this in perspective, even the consumption of the recommended five daily servings of fruits and vegetables may provide just 200 mg of vitamin C, whilst even commercial multi-vitamin supplements typically provide only around 60 mg.

But the real story's actually even worse than this, not only because all too few people actually manage to consume the recommended quantities of fruit and vegetables, but because the nutritional quality of those that we do consume is poor, and getting worse.

As long ago as 1936, Senate Document 264 noted that the poor quality of American farm soils was leading to widespread nutritional deficiencies, and the 1992 Earth Summit reported that mineral concentrations in US farm soils were 85% lower than those of a hundred years ago.

Nor does the problem lie just in the soil. The modern Western diet's preference for highly refined grains, and the treatment of fruits and vegetables with preservatives, dyes, pesticides and even radiation is a proven disaster for vitamin and mineral retention, as well as a significant toxic assault on the body.

Indeed, the problem for us in the twenty-first century is that our environment seems as though it might have been expressly designed for ill-health. Daily we're exposed to a kind of toxic soup of pollutants such as industrial emissions, car exhausts, pesticides, herbicides, dyes and all kinds of everyday household chemicals. But as the liver works ever harder in an increasingly desperate struggle to detoxify the body, an unwelcome side effect is that it produces enormous quantities of the free radicals which are amongst the chief contributors to premature ageing and degenerative disease.

In these adverse circumstances it can only make sense to ensure that the body is as lavishly provided as possible with the top quality anti-oxidants of which vitamin C is certainly one of the most important.

Fortunately, high dosages of vitamin C are readily available as supplements and fortunately, too, it seems that the manufactured kind of l-ascorbic acid (vitamin C) is chemically identical to that obtained naturally. This is not in any way to deny the importance of healthy eating, or to suggest that it doesn't make sense to try and eat the recommended five daily servings of fruit and vegetables, because these contain a myriad of trace nutrients which operate synergistically within the body, and which all need to be present for optimum health.

It does suggest, however, that to rely on diet alone, however apparently healthy, may well be to risk missing out on the vital anti-oxidant properties of vitamin C which may in time be reflected in chronic, degenerative, if not acute, disease. It should be noted as well, that as a water-soluble compound, any excess vitamin C is harmlessly excreted by the body.

So with all the health benefits of vitamin C at stake, it surely makes sense to err on the side of taking in too much rather than too little.

Nine Anti-aging Tips to Stay Younger Longer

Can't figure out where to start to improve the quality of your lifestyle? Here are some simple things to help get started. If you can follow the suggestions for a month, you will be well on your way to reducing belly fat AND establishing an anti-aging lifestyle.. It's not difficult – if you really want to do it. Remember, these are baby steps, but you have to start some place.

1. Check your fridge. If you use butter, you can use less by diluting it half-and-half with olive oil. Let a bar of butter soften to room temperature. Beat it smooth and gradually beat in an equal amount of olive oil. It will be runny but will set up to a soft spread in the fridge.

2. Rethink the mayo. If you can't live without it, make your own with olive oil. Then you will really have something worthwhile to spread on bread or to make a salad.

3. If you eat peanut butter, buy Smart Balance peanut butter for it's omega- 3 content. If you are not taking an omega-3 supplement, you should.

4. Keep a gallon bottle of water on the kitchen counter or at work. Drink at least half of it every day. Having it in plain sight will help you drink more of it.

5. Check your pantry. Don't restock chips, cookies, soda, toaster pastries, sugary breakfast "cereals" (a misnomer if ever there was one!) It's just as quick and easy to scramble a couple of eggs for breakfast as it is to wait for a pastry to toast. I know how tough it is to get rid of the nutritionally bankrupt junk, so do it in small steps if you can't do it cold turkey.

6. Substitute whole grain bread for white bread. Beware of "wheat" bread – it's probably not whole wheat. If possible, go to a whole foods market and get some real whole wheat bread. In my area, I can buy incredibly good whole grain bread from Alvarado Street Bakery. You can buy it online, and if you do, put it in the freezer the minute it arrives. Because it's "live" and preservative free, it will get moldy very quickly if you leave it out. I particularly enjoy the Alvarado sprouted whole grain bread.

7. Having a whole foods market nearby makes it easy to cut down on processed food. If you make a trip a couple of times a week you can get all the super-fresh veggies and fruit you will need to provide a healthier diet for your family. For protein, focus on chicken – preferably baked or roasted. I use a lot of ground turkey – you can prepare it so many ways. If you work and are pressed for time, many whole foods markets offer excellent take out.

You will notice I did not mention fish. I rarely buy fish because it's so difficult to get good fish. If I can't get wild salmon or other fish that hasn't been farm raised, I don't bother. I would never eat shrimp because it causes too many intestinal problems. There is another way to get the benefits of eating fish – in fish oil capsules. True, you run the risk of contamination there as well. But I think I'd rather opt for the capsules than supermarket fish that looks like it's seen better days, and probably has.

8. Skip fried anything as much as possible. And you certainly won't need your deep fat fryer anymore, so throw it away. (Well okay, give it away. But you won't be doing anyone a favor.)

9. If you are an emotional eater, eat unbuttered popcorn or dry roasted nuts, such as almonds or cashews. You can flavor popcorn with garlic salt or any seasoning of your choice. Or try spritzing on garlic flavored olive oil. Drink green tea to wash it down. Most commercial green teas are bitter or tasteless but I have found some very tasty teas at Clipper teas.

This should be more than enough to help you start to live a healthier, anti-aging lifestyle. Start with at least one step and add others as you develop a system.

The value of proper sleep should also never be underestimated. A good night’s sleep also helps with boosting immune system functioning. In the busy world of long work hours and the demands of home and family it can be difficult to get enough sleep each night. You can feel rundown and drained of energy, which I'm sure we all have certain times of our lives. When a person is lacking enough sleep they become much more susceptible to contracting illness and disease.

Boosting immune system function can be done with just a few steps. Your immune system will respond to a healthier diet, exercise and a reduction in stress. There are absolute ways in which you can help yourself but it must start with you. The responsibility is yours.

Low-carbo, high-protein diets increase cholesterol

Now, anybody who wants to lose weight knows Atkin diets.

Sure, they are said to be effective in losing weight easy and fast, which is exactly what most of us dream of.

But hey, a report in the Archives of Internal Medicine pointed out that low-carbohydrate, high protein diets are found to increase our cholesterol levels. And we are referring to both LDL (low density lipoprotein, which is also known as "bad" cholesterol) and HDL (high density lipoprotein which is also the "good" cholesterol).

HDL helps the body get rid of bad cholesterol. LDL is said to cause build-up of plague on the artery walls, contributing to risks of cardiovascular diseases.

The findings were from an analysis of the five clinical trials comparing low-fat diets with low-carbohydrate diets. These trials attracted 447 participants whose ages range from 42 to 49 years old. Out of 447 participants, 225 are on low-fat diets while the rest are on low-carbo diets.

Both groups were then tested for their weight loss and blood pressure after six and nine months' period. It was expected that for the first 6 months, the participants on the low-carbo diet had lost more weight. After 12 months, the outcome of weight loss and blood pressure turned out to be the same for both groups.

What raises our concern was that from the results of low-carbo diet group, total cholesterol increased throughout the 12 months period. A low-carbo diet is usually high in fat, resulting in a higher level of cholesterol.

However, the results further show that a higher cholesterol level had led to a concurrent increase in both LDL and HDL. They also had a lower level of triglycerides, a type of fat, which LDL carries in the blood.

So does the beneficial effects of low-carbo diet appear to have on HDL and triglyceride levels cancel out their apparent negative effects on overall and LDL cholesterol levels?

Not necessarily so, as another study published in the Journal of Lipid Research in 2003 showed that high triglyceride levels, high LDL levels and low HDL levels are actually independent risk factors for cardiovascular disease.

So despite what the low-carbo diet results show, more evidence is needed to support the conclusion as the study had its limits. The researchers in the low-carbo diet report also expressed concern about the effects of the low-carbo diet on cholesterol levels and the cardiovascular system as it contains large amounts of fats and proteins.

This is because you are more likely to consume more foods high in saturated fats when you turned to high-protein diet. This increases your calories as fat provides more calories per gram than carbohydrates. Fat contains nine calories per gram while carbohydrates provide four calories per gram.

You are putting yourself at a higher risk of contracting coronary heart disease, if you do not reduce your fats intake.

Other health problems associated with a diet high in protein include kidney failure, high cholesterol levels, osteroporosis and kidney stones. As a guide, an adult should obtain 50 to 60 percent of their calories from carbohydrates, 10 to 15 percent from protein and 20 to 30 percent from fats.

Therefore, it is not advisable to go long term on low-carbo, high-protein diets.

Stress-relief: Techniques That Work For You!

Stress can come in many forms - from workplace tension, marital woes, family squabbles, financial perils - only to name a few. Stress triggers your body's "fight or flight" response either enabling you to flee from harm (as in escaping a potentially harmful situation) or, more likely in our modern times, it triggers a response to a given situation or event, such as an argument, traffic jams, deadlines, etc. These situations increase our blood pressure and breathing as if we were to flee.

Chronic stress keeps our body in this "fight or flight" reaction thus leaving it at an unhealthy level that negatively affects our immune system. In fact, studies have shown that stress is related to 90% of all illnesses.

How do you recognize stress? Stress has both physical and emotional factors with each presenting their own list of symptoms, such as an inability to sleep, headaches, pain in back, shoulder, or neck areas, trouble thinking clearly, lack of concentration, and memory loss.

Learning to handle stress is essential to good health. Long-term stressful situations lend themselves to obesity, heart disease, diabetes, cancer, tooth and gum disease, hyper-thyroidism, and other major health issues.

What techniques can you use to help you to cope with stress?

* Have a distraction free bedroom by removing the TV and all work-related items from the room.

* Avoid staying in your bedroom when not sleeping or relaxing; use only at bedtime.

* Use relaxation exercises to learn how to relax, such as the one below, even while at work during a break or lunch.

* Eat right and exercise regularly, even if it is only 30 minutes a day three or more times a week.

* Plan your daily activities according to priorities. Create a check list.

* Mentally or physically check off completed tasks which will give you a sense of accomplishment.

* Participate in wellness programs or create one of your own by receiving a weekly massage, pedicure, body wrap, or another relaxing treatment.

If your stress is due to a major crisis as in disasters caused through nature, war, or an act of violence, professional help may be needed to learn to cope with the multiple emotional and physical issues involved. Consult with your health care provider about your stress factors.

Relaxation Exercise that you can use anywhere:

*Close your eyes.

*Take a deep breath.

*Inhale and exhale slowly throughout this whole process.

*Concentrate on your toes.

*Will your toes to relax.

*Next concentrate on the bottom of your feet.

*Relax the bottom of your feet.

*Continue until you work your way up to the top of your head.

What Doctors Don’t Tell You about Your Allergic Rhinitis

As a medical student I have access to the same clinical reference databases your doctor uses. One of the most widely used databases, called UpToDate, reviews all the pertinent literature and then develops treatment guidelines for various diseases and conditions.

As I was perusing the Allergic Rhinitis section, it struck me that there was some important information in here that doctors probably weren’t telling their patients. I believe it’s important for everyone to be fully informed about their own health so I’m going to go ahead and share this information with you.

1. If you must use nasal decongestants make sure you only use them for a week at the very most:

Nasal decongestant sprays are not recommended in the treatment of allergic rhinitis. Tachyphylaxis (rapidly decreasing response to a drug following administration of the initial doses) develops after only three to seven days. Then rebound nasal congestion results, and if you continue to use the nasal spray anyways it causes rhinitis medicamentosa. Which is literally rebound nasal congestion caused by decongestants. Even worse, the swelling of the nasal passages may eventually result in polyps that need to be surgically removed.

2. Use a saline nasal spray or wash before your topical nasal medications:

Topical nasal corticosteroids are now the first line of treatment for allergic rhinitis. Intranasal steroids are more effective than cromolyn and second generation antihistamines in improving the symptoms of allergic rhinitis and the symptoms of seasonal asthma. BUT treatment failures may occur if mucus or other debris is not cleaned from the nose before application. Saline nasal sprays or washes should be used to clean the nasal passages before you use your meds.

3. Save your money! You might not be a good candidate for allergy shots:

A large number of controlled studies have demonstrated that allergen immunotherapy to tree, grass and ragweed pollens, mold and house dust mite are effective in the treatment of allergic rhinitis. But if you are on beta-blockers (often used in the treatment of high blood pressure and heart problems) you aren’t a candidate for allergy shots.

If you aren’t on beta-blockers and are willing to try a 2 year minimum (normally 3-5 years) trial of allergen immunotherapy then consider this as well. Individuals who don’t have easily demonstrable quantities of allergen-specific IgE (antibodies) by prick testing are not candidates for allergy shots. So you may save yourself a great deal of time and money by making sure your doc checks your IgE levels first.

If you decide against the allergy shots, but aren’t enamored with the thought of taking symptom-suppressing medications for the rest of your life then you might try some alternative treatments for allergic rhinitis. Homeopathy is an alternative field of healthcare which focuses on restoring the health of the individual rather than suppressing the symptoms of their disease.

In particular, an encouraging study was done on individuals living in the southwest who suffered from seasonal allergic rhinitis. The participants were given a homeopathic regional allergy mix (a medicine prepared from allergens specific to the southwest that in its final form is incredibly dilute). The study showed significant reduction of symptoms and improvement in quality of life over a 4 week period in the homeopathic group compared with the placebo group.

There are homeopathic medicines available that contain the appropriate allergen complex for any area of the U.S. The allergen complex most often contain specific grass, trees, weeds, pollen and mold that are found in high concentrations in specific regions. These regions include:

Southern U.S.- AR, LA, MS, AL, GA, FL, TN, KY, WV, VA, NC, SC, & east TX

Rocky Mountains- CO, ID, MT, UT, WY, & NV

Southwestern U.S.- OK & TX

Desert- UT, CO, CA, & NV

Northeastern U.S.- ME, NH, VT, MA, CT, RI, NY, NJ, PA, DE, MD

Plains- ND, SD, NE, & KS

Pacific- WA, OR, & CA

Great Lakes- MN, WI, MI, IA, MO, IL, IN, & OH

Don’t despair if your allergic rhinitis is caused by ,other than those mentioned above. There are also homeopathic formulations which contain allergen complexes such as animal hair & dander, and dust, mite & roach. These 2 causes of allergic rhinitis have higher remission rates than pollen allergy and may respond even more favorably to a homeopathic attempt at allergy correction.

Thyroid Surgery and Thyroidectomy

The following article looks at the various issues surrounding thyroid surgery, known as thyroidectomy.

Reasons for Thyroid Surgery

Thyroid surgery is performed in a number of circumstances:
  • as a treatment for thyroid cancer
  • when an enlarged thyroid (goiter) or multiple nodules cause cosmetic, breathing or swallowing problems
  • in a pregnant woman, when her hyperthyroidism is not controllable by antithyroid drugs, and requires immediate treatment
  • when other forms of treatment for hyperthyroidism -- i.e,. antithyroid drugs or radioactive iodine have not been effective. (This is applicable in the U.S. Outside the U.S., surgery is sometimes performed as a hyperthyroidism treatment before or instead of radioactive iodine.)
  • in children, if the practitioner or parent wishes to avoid radioactive iodine
  • when the patient refuses antithyroid medications or radioactive iodine
  • when a patient wants to try to get pregnant quickly after treatment

Choosing a Surgeon

Complications are more likely with surgeons who have less experience performing thyroid surgery, so make sure that your surgeon has extensive experience in thyroid surgery. You can find out more in the article about Finding a Top Thyroid Surgeon.

Types of Thyroid Surgery

There are three main types of thyroid surgery:
  • Total Thyroidectomy -- Complete Removal of the Thyroid
    This is the most common type of thyroid surgery, and is often used for thyroid cancer, and in particular, aggressive cancers, such as medullary or anaplastic thyroid cancer. It is also used for goiter and Graves'/hyperthyroidism treatment.
  • Subtotal/Partial Thyroidectomy -- Removal of Half of the Thyroid Gland
    For this operation, cancer must be small and non-aggressive -- follicular or papillary -- and contained to one side of the gland. When a subtotal or partial thyroidectomy is performed, typically, surgeons perform a bilateral subtotal thyroidectomy which leaves from 1 to 5 grams on each side/lobe of the thyroid. A Harley Dunhill procedure is also popular, in which there's a total lobectomy on one side, and a subtotal on the other, leaving 4 to 5 grams of thyroid tissue remaining.
  • Thyroid Lobectomy -- Removal of Only About a Quarter of the Gland
    This is less commonly used for thyroid cancer, as the cancerous cells must be small and non-aggressive.
The issue of a subtotal/partial, vs. total thyroidectomy is controversial. Some practitioners prefer to perform a partial thyroidectomy whenever possible, believing that they will leave behind enough thyroid tissue to prevent hypothyroidism. (A total thyroidectomy has nearly a 100 percent chance of causing hypothyroidism). The risk of hypothyroidism with subtotal thyroidectomy is, however, quite high, and some experts say that more than 70 percent of patients receiving a subtotal thyroidectomy will become hypothyroid. Since one of the main reasons for subtotal thyroidectomy is to prevent hypothyroidism, and that goal is achieved in only a minority of cases, experts increasingly believe that there is no added benefit to subtotal thyroidectomy, and are more routinely recommending a total thyroidectomy.

What You Are Likely to Experience

In most cases, surgery of the thyroid is not highly complicated, and usually takes no more than two hours.

Removal of half of the thyroid takes 45 minutes to an hour, so if the entire gland is being removed, the surgery will last about an hour and a half.

Check with your surgeon about medications you are taking, and what you should/shouldn't take in the days prior to surgery.

You will most likely be asked to check into the hospital the morning of your surgery. Typically, your surgeon will ask that you refrain from eating or drinking after midnight the night before surgery.

Outpatient or Hospital Admission?

Depending on the condition of the patient, an overnight or two-night hospital stay may be planned. Outpatient thyroid surgery is becoming increasingly popular, however, and research shows that outpatient thyroid surgery is safe, effective -- and less expensive -- for most patients, and may be preferable to traditional inpatient hospital stays.

General or Local Anesthesia?

Thyroid surgery is more commonly performed with general anesthesia. Some surgeons are now using local anesthesia, plus a sedative, however, to perform thyroid surgery.

The benefits of local anesthesia are that it is associated with a shorter hospital stay, shorter actual surgery time, and less vomiting and nausea after surgery.

If you choose local anesthesia, your doctor will typically give you numbing medication for the thyroid area, plus a mild sedative to help you stay calm. You will, however, be awake during the surgery, and able to interact with your surgeon.

Not many surgeons are trained to do thyroid surgery under local anesthesia. So if you want to proceed with this option, be sure your surgeon has done a number of thyroid surgeries with local anesthesia. (Some experts suggest you look for a surgeon who has performed this procedure at least 50 times.)

The Surgical Procedure

In the surgery, the surgeon will cut a 3- to 5-inch incision across the base of your neck in front. The skin and muscle are pulled back to expose the thyroid gland. The incision is usually made so that it falls in the fold of the skin in your neck, making it less noticeable.

Blood supply to the gland is "tied off," and the parathyroid glands are identified (so that they can be protected). The surgeon then separates the trachea from the thyroid, and removes all or part of the gland.

An In-Depth Look

A newer technique, known as endoscopic thyroid surgery, involves using a small magnifying camera inserted in the neck. Carbon dioxide gas is pumped into the neck area to help make it easier to see and work on the gland. A second small incision is made, and a thin tube with a scalpel-like edge is inserted through that incision. This tube is the surgical tool that is used to remove the thyroid. Endoscopic surgery, because it involves two small scars of less than one inch, usually leaves less visible scarring, and allows a quicker return to normal activity. Sometimes, the entry point is under the arm -- known as axillary surgery.

Endoscopic surgery is not common, however, and you'll need to investigate to find a surgeon with experience doing these surgeries.

Most surgeons use dissolvable stitches, but you may want to ask your surgeon ahead of time which kind he plans to use, because the non-absorbable stitches actually tend to cause less scarring. If you have any history of allergic skin reactions to past stitches, you may also want to ask your doctor about using hypoallergenic suture material.

After the surgery, you will usually remain under observation at the hospital for at least 6 hours. If you are having outpatient surgery, you may be discharged after that point.

Before you are discharged, your incision is usually covered with a clear protective waterproof glue called colloidium. (This allows you to bathe or shower after the surgery.)

Rarely, if there is concern about bleeding or if the thyroid is very large and the surgery has left a large open space, a drain will be left in your wound to prevent fluid from accumulating. You'll need to return to the surgeon a few days later to have the drain removed.

After Your Surgery

Thyroid surgery is generally considered extremely safe. There are some common short-term side effects after thyroid surgery such as pain when swallowing, and neck stiffness. Most patients also become hypothyroid after surgery and require thyroid hormone replacement therapy. While complications are not common, a few can result from thyroid surgery. These include hypoparathyroidism and hypocalcemia, and laryngeal nerve damage. Signs can include numbness and tingling around your lips, hands, and the bottom of your feet, muscle cramps and spasms, bad headaches, anxiety, depression, hoarseness, and difficulty speaking loudly. If you saw a tiger, you'd run for your life. But what if you didn't see it? For those of you with type 1 or type 2 diabetes, heart disease is that unseen tiger. You're two to four times more likely to develop a heart condition than people without diabetes? Even more shocking, those individuals are more likely to die from heart disease or other cardiovascular ills than from the complications of diabetes itself. Yet surveys show that 68 percent of Americans with diabetes are unaware of their increased cardiovascular risk.

If you have diabetes, now is the time to take steps to protect your heart.

"High blood glucose alters cell metabolism," says Richard Kahn, Ph.D., chief scientific and medical officer of the American Diabetes Association (ADA). When the function of platelet cells changes, the likelihood of blood clots increases and the risk of heart attack rises. Cells that line the artery walls are also affected; the ability of blood vessels to dilate is impaired, which can lead to high blood pressure.

One pivotal phenomenon underlying these changes is inflammation, already known to promote heart disease. Researchers at the University of Virginia School of Medicine in Charlottesville, for example, found high blood glucose linked to chronic inflammation in the blood vessel walls of mice.

Prevention Works

The first step to protecting your heart? Control your glucose. In the groundbreaking Diabetes Control and Complications Trial to determine the effects of glucose on complications, type 1 patients underwent intensive glucose control. A later study found these participants were 57 percent less likely to die from cardiovascular disease over the next 16 years.

Although the findings haven't been confirmed in those with type 2, high blood glucose levels have been proven to increase the risk of heart disease in both kinds of diabetes. Says David Nathan, M.D., professor of medicine at Harvard Medical School and co-chair of the type 1 study, "The major difference is that people with type 2 are generally older, heavier and more likely to have hypertension and abnormal cholesterol levels, all of which puts them at much higher risk for heart disease." For them, prevention is even more urgent.

Keep your heart healthy by following the ABCs of diabetes prevention: Aim for A1C levels below 7 percent, blood pressure below 130/80 mm Hg and LDL cholesterol below 100 mg/dl.

"Everyone with diabetes should know their numbers and goals, and get checked regularly," says John Buse, M.D., lead author of the ADA and American Heart Association's new joint statement on preventing cardiovascular diseases in people with diabetes. Start with a healthy lifestyle:

  • Nutrition Eat less fatty red meat, cheese, butter, processed foods and fast food; instead, choose more beans, vegetables, fruits and whole grains, along with lean meats and poultry, seafood, low-fat dairy and nuts. If you're overweight, cut some 250 to 500 calories a day until you've lost about 7 percent of your weight. "To do this right, ask your doctor to refer you to a registered dietitian," Dr. Buse says.
  • Exercise Each week, be sure to do a minimum of 150 minutes of moderate-intensity aerobic activity--swimming, biking, walking briskly. Exercise at least every other day.
  • Even if your doctor prescribes medication, don't give up on diet and exercise: A healthy lifestyle not only has broad positive effects on blood glucose levels, blood pressure and cholesterol levels, says Dr. Buse, but also helps you fight depression, function at your best and maintain your energy.

    Diabetes insipidus is sometimes called water diabetes and is completely different to diabetes mellitus (sugar diabetes) which has just been discussed.

    Diabetes insipidus occurs when the pineal gland in the brain is not producing enough of the chemical ADH (anti-diuretic hormone). This chemical is produced by the pineal gland in the brain and is carried by the blood stream to the kidneys where it stimulates the kidneys to reabsorb water in the process of producing urine. Water is a precious commodity for the body and thus the water in the body is constantly being purified and recycled by the kidneys. Very little water is released from the body by the kidneys in the form of urine. The ADH is produced by the pineal gland in the brain which acts like a control centre for regulating the amount of water in the body. The ADH is simply a messenger chemical which is produced by this control centre (the pineal gland) to tell the water-recycling factory (the kidneys) how much water should be recycled and reabsorbed. If the body has too much water (over hydrated) less ADH is sent to the kidneys which then produce more urine and thus the excess water is expelled. If the body needs to conserve water or is starting to dehydrate then more ADH is sent to the kidneys which then produce less urine and water is conserved.

    In the case of diabetes insipidus, the pineal gland does not produce enough of this chemical and thus the kidneys think that they are being told to recycle less water and allow more of it to leave the body in the urine.

    Most human beings experience this same effect when drinking alcohol. This is because alcohol interferes with the messenger chemical ADH and thus the ADH does not tell the kidneys to recycle water. The kidneys then release more water in the urine and thus we have to urinate more often when drinking alcohol.

    The symptoms of diabetes insipidus are simply drinking more and peeing more. The affected dog will feel fine and act normally. The primary problem is that the kidneys are producing too much urine because the body wants to avoid becoming dehydrated and thus needs to replace the lost water, this will increase the patient's thirst levels. Some dogs may even seem to become incontinent and start wetting indoors and even start wetting their beds. Dogs aslo have various health problems as the human body.

As more companies compete for your business, the opportunities to save money will continue to grow.

    Since the advent of employer-sponsored health insurance during World War II, the pricing and quality of medical services has been shrouded in mystery. As the real price for computers, organic food, big-screen TV's, and just about everything else has continued to drop (including health services where there is not a third party payer, such as laser eye surgery, contact lenses, over the counter medication), the price of healthcare has continued to rise. But that is finally beginning to change. And you, fellow HSA-owners, are the reason.

    To gain a competitive advantage, healthcare providers have traditionally attempted to grow local market share in an attempt to extract higher payments from insurance companies. But since HSA owners have the ability to accumulate unspent funds and invest them tax-free, those of us with a Health Savings Account have a strong incentive to avoid unnecessary care, and to be more cost conscious when we seek treatment.

This uses sonar to test for fluid behind the eardrum, which may indicate an infection. "The QuickVue Strep Test" which costs less than $4 per test in a pack of 25, can quickly help you determine if your child has a strep infection, which would require a doctor's visit, from a common viral infection, which would not.

    Demand Price Transparency

    Health Savings Accounts reward personal responsibility in three ways: 1) they reward you with tax-breaks for putting money aside to cover future medical expenses; 2) they reward you for taking care of your health by enabling you to grow your account; and 3) they reward you for being a cost-conscious and discerning consumer.

    So be a discerning consumer, and spend your money wisely. Remember that the doctors and healthcare providers you see work for you. If you don't get the quality of service or a fair price, take your business elsewhere. Here are some common sense suggestions to make sure you do get a good price:

    1. Ask how much it will cost, before you buy. There is nothing else that you buy without knowing the price up front, so don't feel intimidated to ask your doctor the same.

    2. Review your bill before paying it. You might be shocked how often extra charges are "accidentally" tacked on to hospital bills.

    3. Ask for a cash discount. To avoid the hassles of filing for insurance and trying to collect past-due charges, most physicians will gladly offer a cash discount if you ask.

    4. Explain that you will be paying out of your own pocket. When a doctor is prescribing tests or writing prescriptions, he or she is rarely taking cost into consideration. The American Journal of Preventive Medicine recently reported that up to $63 billion in medically unnecessary tests are ordered every year.

    5. Vow never to pay list fees. Doctors and hospitals routinely discount their services to insurance companies and PPO organizations. As a cash-paying customer, you should get the best price available.

    For many years, a small group of health economists and other policy-makers pushed for a more market-based approach to healthcare. They correctly argued that healthcare was like any other market and that if you put a true price on health care services and let the market function, costs could be controlled. We are now beginning to see this happen.

    Our healthcare system is the best in the world. It is a dynamic and complex work in progress, which can only get better as the consumer gets involved. So be savvy about how you spend your healthcare dollar. And watch those unspent funds in your Health Savings Account continue to grow.

    The Medicare Trust Fund will soon be out of money, and there will be no practical way for the government to continue to provide the level of benefits that current Medicare recipients receive.     Who Gets Migraine

    It’s not clear why some people develop migraines and others don’t. If both your parents are migraine sufferers, there’s a 75 percent chance you’ll be one, too. If only one parent is, your risk drops to 50 percent. And if a distant relative complains of the headaches, there’s still a 20 percent chance you could end up a migraineur. (That’s the term used to describe a migraine sufferer.) Your gender may predispose you to migraines as well: Women are three times more likely than men to get this type of headache.

    What Happens

    “The brain of a migraine sufferer is inherently more sensitive than the brains of others,” says Fred Freitag, D.O., associate director of the Diamond Headache Clinic in Chicago—and himself a migraineur. “When this sensitive brain gets bombarded by outside and inside stimuli, cells in the cortex of the brain become hyperexcited and start a process of electrical discharge.” As electrical impulses spread across the surface of the brain, chemicals that dilate blood vessels and inflame surrounding tissue are released and kick into action. These chemicals trigger a series of events that eventually produce the throbbing pain (usually on one side of the head), nausea, and sensitivity to light and sound that are hallmark migraine symptoms. Before the headache comes on, about one third of sufferers also experience an aura—flashes of light, blind spots or zigzag lines, or tingling in the face and arm.

    What Can be Done

    Avoiding triggers is the first step to heading migraines off. Headache experts usually have migraineurs keep a diary for two to three months to identify foods, external factors and behaviors that might be making their heads throb. Then they’re ready for action. “If you that know certain foods give you a headache, avoid them,” says Dr. Freitag. Also, get on a schedule. Eat at regular times and go to bed and get up at the same time every day. If you go to bed at 11 and get up at 6 all week but sleep in on Saturday, your blood sugar will drop because you’re eating breakfast later. In addition, your internal clock gets thrown off. “Both can stimulate a sensitive brain,” says Dr. Freitag.

    Eat a healthy diet. Magnesium-rich foods, such as whole grain breads and pasta, may help stave off migraines. And exercise regularly. Even 20 minutes of aerobic exercise a few times per week stimulates the production of endorphins, the brain’s natural painkillers. Relaxation training, acupuncture and massage can help, too. If these measures aren’t enough, ask your doctor whether you are a candidate for medication. The good news: With a little trial and error, experts say, you can usually find relief.

    Trigger Effect

    Experts have identified a slew of stimuli that can cause migraine attacks. These range from preservatives to perfumes to changes in barometric pressure. In women, hormone levels are often to blame. “Their hormones peak, drop during ovulation and then climb again, and then fall just before menstruation,” says Robert Kunkel, M.D., a consultant in the department of neurology at the Cleveland Clinic Headache Center. “It seems to be the fall of estrogen levels in the blood that excites brain cells, not hormone levels per se.”

    A human embryo is obtained when a woman’s egg is fertilized by a man’s sperm. This occurs in the human body but it can also be done in a laboratory. The procedure is often used in cases of infertility.

    I have three grandchildren formed in this way. Their mother donated eggs, their father donated sperm, and the technicians watched the fertilization take place under a microscope. The fertilized eggs were placed in the mother and she gave birth to triplets.

    If embryos formed in this way are not placed in the mother they can be and are used for medical research. Often extra fertilized eggs are produced during this process. Scientist would like to harvest these extra eggs rather than discard them. They could then use them to obtain stem cells.

    Stem cells are never obtained from fertilized eggs that reside in a woman’s body.

    The embryos obtained after they are a few days old are in the form of a mass of cells called a blastocyst; the embryo of about 150 cells. The blastocyst consists of a sphere made up of an outer layer of cells (the trophectoderm), a fluid-filled cavity (the blastocoel), and a cluster of cells on the interior (the inner cell mass).

    How are stem cells obtained?

    Cell cultures are grown in the laboratory by transferring the inner cell mass of about 30 cells into a culture dish which has a nutrient broth. The cells quickly multiply and fill the dish. They are then transferred to other culture dishes and the process goes on for months.

    Once the cells are obtained they can be frozen and shipped to other laboratories.

    What other potential stem cell sources are there?

    Adult stem cells are a potential source. They can be used to reproduce cell of their type. That is, while embryonic stem cells can differentiate into any type of cell, adult stem cells can only reproduce cells of their type. If they are muscle cells, they can be used to reproduce only muscle cells. However, recent work has indicated that some adult stem cells may be able to differentiate into other cell types.

    Why are we and special groups interested in stem cells?

    Because stem cells can differentiate, that is, can be used to reproduce other cell types, they have tremendous potential for solving many human health problems.

    Some groups do not want scientist to take human embryos for research in any way whatsoever. Because of this, President Bush restricted stem cell research to existing stem cell sources. Other governments have stayed out of the research arena and stem cells are collected at the whims of the scientist.

    Scientists argue that excess stem cells are produced in fertility clinics and that they should be used to benefit mankind.

    What are the goals of stem cell research?

    First, scientists want to understand differentiation. We all know that the human embryo creates all the cell types in the human body. Scientists want to know how and when genes turn on and off to create a particular cell type. Abnormal cell divisions cause birth defects and cancer. Scientists want to know what signals a change in the process of cell development. This could lead to cures for cancer and birth defects.

    Stem cells could be used to test new drugs rather than human guinea pigs and animals. Damage to the stem cells would eliminate the drug before it could do damage in the market place, as so many drugs do now.

    Today, donated organs and tissues are often used to replace ailing or destroyed tissue, but the need for transplantable tissues and organs far outweighs the available supply. Stem cells, directed to differentiate into specific cell types, offer the possibility of a renewable source of replacement cells and tissues to treat diseases including Parkinson's and Alzheimer's diseases, spinal cord injury, stroke, burns, heart disease, diabetes, osteoarthritis, and rheumatoid arthritis.

    Considering Weight Loss Surgery?

    Also, you must understand that you are undergoing the surgery for yourself and for your health. If you and your spouse are having problems and you think that surgical option will make your spouse love you more, then the surgery definitely not for you.

    Another example will be that you want to attract somebody else and think that a slimmer post surgery will make you more attractive to the person you like, then again, this is not for you because you may suffer from bad surgery psychological effect when you fail to attract that person.

    Before your surgery, extensive medical and psychological testing will be conducted by your plastic surgeon to determine if a meet the medical guidelines and is a good candidate for a successful weight loss surgery.

    Do research the subject extensively and consult your plastic surgeon or even a psychologist before you come to an informed decision to undergo a weight loss surgery.

    Get Rid Of That Dandruff

    1. Shampoo everyday. This is one of the easiest ways to do for you to say bye-bye to dandruff and send it down the drain. Shampooing will help rid of those excess oils that often cause dandruff.

    2. Change shampoos. If you have noticed that your regular shampoo is doing nothing to remove dandruff, it is about time that you use an anti-dandruff shampoo. Yet if you can't decide which antidandruff shampoo to use, check the labels and make sure it has any of the following ingredients: Zinc Pyrithione (lessens the fungus), Salicylic Acid (works like a scrub in getting rid of dead skin), Selenium Sulfide (limits cell turnover and potentially lessens the fungus) and Ketoconazole (fights fungi).

    4. Lather for two times. The first lather will remove the flakes and the excess oil build up on your scalp and hair. In short, the first lather simply clears what needs to be cleared so that the second lather works. Leave the second lather in your hair for five minutes before you rinse it. This should be enough time to let the shampoo penetrate your scalp down to the skin cells that need to be dealt with.

    5. Cease those itchy fingers or else you'd end up having wounds in your scalp. But just in case you caused scratches on your scalp, stop using the medicated shampoo for the meantime. It'd be best to use baby shampoo until the scratches are healed.

    6. Minimize the use of hair products. Mousses, gels and sprays contribute to the buildup of excess oils.

    "In the summer and fall of 1989, an epidemic outbreak of eosinophilia-myalgia syndrome (EMS) occurred in the United States. This illness is associated with the use of dietary supplements containing L-tryptophan. In all, more than 1500 cases of EMS, including at least 37 deaths, have been reported to the national Centers for Disease Control and Prevention (CDC), although the true incidence of the disorder is thought to be much higher. Some individuals suffering from L-tryptophan-related EMS have recovered, while other individuals' illnesses have persisted or worsened over time."

    This report was published by the FDA in 2001, after the 37 known deaths. With a number of highly questionable rationalizations and unlawful marketing, L-trytophan is still being sold as a sleep aid in 2007.

    There are hundreds more examples of dangerous all natural and manufactured dietary supplements being sold to help you sleep.

    Breast Cancer Genes in Women Could Come from Dad

    I recall feeling blessed and very fortunate breast cancer genes were not present in my Mother’s family tree. A news report released earlier today gave updated information. An oncologist reported many doctors often overlook the genetic risk from the father's side of the family.
    A deadly gene's path can hide in a family tree when a woman does not have many aunts or a few older sisters, “making it appear that her breast cancer struck out of nowhere when it really came from Dad."
    The study was published in the Journal of the American Medical Association today. It suggests thousands of young women with breast cancer - an estimated 8,000 a year in the U.S. - aren't offered testing to identify faulty genes and clarify their medical decisions. Guidelines used by insurance companies to decide coverage for genetic testing should change to reflect the findings, said study co-author Dr. Jeffrey Weitzel of City of Hope Cancer Center in Duarte, Calif. Testing can cost more than $3,000.
    Dr. Weitzel said, "Interestingly, it's about Dad." He continued by saying, half of genetic breast cancers are inherited from a woman's father, not her mother. But unless Dad has female relatives with breast cancer, the faulty gene may have been passed down silently, without causing cancer. (Men can get genetic breast cancer, too, but it's not common.)
    The study looked at the genetic test results from 306 women diagnosed with breast cancer before age 50. None of the cancer patients in the study had a family history of breast or ovarian cancer.     Cellulite is alleged to be a special type of "fat gone wrong," a combination of fat, water, and "toxic wastes" that the body has failed to eliminate. Cellulite is actually a fancy name for the collection of fat that push against the connective tissue beneath a person's skin, which causes the surface of the skin to dimple or pucker and look lumpy. Cellulite is a permanent change in the fat that's stored under your skin.

    Cellulite
    More and more people are complaining of cellulite. The term cellulite originated in France more than 150 years ago and began appearing in English language publications in the late 1960s, the earliest reference in Vogue magazine, "Like a swift migrating fish, the word cellulite has suddenly crossed the Atlantic.

    Cellulite -

    Profile Cellulite is a common term used to describe superficial pockets of trapped fat, which cause uneven dimpling or orange peel skin. Although cellulite becomes more noticeable with age, largely because the skin gets thinner overtime, it generally strikes individuals in their 30s. Women all over the world are bothered by their cellulite as is evidenced by the millions of dollars that are spent each year on treatments that claim to reduce cellulite.


    Treatments like liposuction (surgery to remove fat) and mesotherapy (injection of drugs into cellulite) are either expensive or may produce only temporary improvement. Liposuction, which extracts fat from under the skin, is not effective for cellulite reduction. Whatever the cause of cellulite, it's important to know that there aren't any miracle products, treatments, or medicines that can make it go away.

    The only known way to reduce the appearance of cellulite is to reduce overall body fat by altering diet or exercise, although it may remain visible even on very slim women. Experts agree that an exercise routine that combines aerobic exercise with strength training is the best weapon against cellulite.

    Cellulite tends to be a little bit less noticeable on darker skin. Dieting has to be combined with regular exercise to be effective in controlling or reducing cellulite.

    Fat

    From an anatomical point of view, cellulite consists of swollen fat cells in certain problem areas. It can be a result of lack of exercise, particularly to the extent that the body cannot illiminate toxins, burn fats and stimulate cell growth when the circulation is not stimulated to it's maximum. After all cellulite is just fat right.

    Diet
    Dieting has to be combined with regular exercise to be effective in controlling or reducing cellulite.
    And while results won’t happen overnight, a consistent program of regular exercise combined with a sensible diet can go along way toward not only improving your appearance, but your overall health as well. Maintain a balanced and varied diet, lots of antioxidants, switch dairy to soya, exercise as hard as your body will allow and keep it up, moisturise and exfoliate (your skin needs attention).
    Eat a low-fat, low-sugar, high-fiber diet with emphasis on fruits, vegetables, and whole grains. Drink plenty of water, up to two liters a day. This will help to assist your body in getting rid of unwanted toxins and waste.

    The good news is that it's not a disease or illness in the real sense of the word, but a sign that your lifestyle is unhealthy.


    Appearance

    The only known way to reduce the appearance of cellulite is to reduce overall body fat by altering diet or exercise, although it may remain visible even on very slim women. Since it's fat, if you lose weight, then the cellulite should improve in its appearance if you're losing in that area.


    Cellulite is not related to being overweight; average and underweight people can also get cellulite. Cellulite is a normal human variation that is viewed by a segment of the public (with the help of the media, some physicians, and salesmen of various sorts) as some sort of a disease. It's not proven whether any cosmetic lotion, massager or pills can reduce/increase cellulite versus ordinary fat.

    Prostate Problems - Symptoms Causing Concern

    Some blood in the urine… burning sensations… or even inability to urinate… all relatively serious problems which might also implicate the prostate.

    BLOOD IN THE URINE

    Just a few drops of blood can make the urine held in your bladder turn a red color. We’re talking about haematuria. Don’t panic – there’s no danger – the presence of blood is more disturbing than dangerous. Drink a lot of water to rince it out then go and see your doctor.
    Haematuria may be due to one of the following:

    • simple congestion of the prostate
    • a unirary infection inflaming the wall of the bladder and making it bleed
    • stones in the bladder which are irritating the wall
    • following some strenuous exercise

    When the prostate is the cause, during urinating the blood will usually arive first followed by clearer urine. Such haematuria might occur from time to time and are not dangerous. However, if frequent, surgical intervention on the prostate might one day be required.

    But note that, if you do have blood in the urine, it is of vital importance that its origin is determined. This will require a thorough urological examination including an ultrasound scan, intravenous pyelogram and fibrescopy of the bladder to determine or eliminate possible other causes (such as a bladder tumour or stones, or a kidney or ureteral tumor).

    A BURNING SENSATION

    If you feel a burning sensation while urinating, this is probably due to a urinary infection which is irritating the bladder or urethra. The burning is often accompanied by a frequent desire to urinate – up to a dozen times a night. However the urine flow will be week since the bladder hasn’t had time to refill. Occasionally the irritation will be so great that the bladder might bleed. You might also have a fever which can signify that bacteria have penetrated the prostate (with associated risk of septicemia).

    Such a urinary infection is diagnosed by a urine analysis and is rapidy treated with antibiotics. For urinary infections with fever, the treatmant will last about three weeks.

    ACUTE URINE RETENTION

    You haven’t urinated for over 24 hours, the bladder is full, blocked; you want to urinate, but can’t; your lower abdomen is distended and painful to the touch. Yes, the bladder is capable of holding over a litre (2 pints) of urine! This is acute urine retention.

    There’s no point in trying to force it or waiting. There’s only one thing to do: insert a probe into the urethra and empty the bladder. You’ll have immediate relief. Contact your own doctor or go immediately to your local hospital emergency department. The probe will remain in place for a few days to enable the bladder to relax. When it’s removed you’ll have about a 50% chance of returning to a normal urination frequency. But beware of any recurrence.

    Acute urine retention can indicate prostate hypertrophy as yet undiagnosed due to the absence of other symptoms. It can also occur in someone undergoing prostate treatment, where it would indicate an evolution in the illness, and surgery might be necessary.

    Once again, you should be aware of other factors which might lead to urine retention, such as taking certain cold and bronchial medicines, constipation, some anaesthetics, and excessive alcohol consummation.

    So there you are. The above three problems are not uncommon, but they are often the cause of much undue worry for those suffering from them. Of course, they should all be reported immediately to your doctor and thoroughly investigated. But I hope that this short article will help to put your mind at rest and that in many cases, your problems might not be so severe or life-threatening as first imagined.

    Aleve, Vioxx, Celebrex, And Bextra: What You Need To Know About Arthritis Pain Medications

    Recent controversy about the safety of pain medications for arthritis has left patients and health care professionals alike confused about which medications are safe to use. In fact, a recent survey by the Boston-based Rippe Lifestyle Institute indicated that many people with arthritis are suffering unnecessarily because they have stopped or reduced their use of pain relievers due to confusion about which drugs are considered safe.

    The survey also showed that now, more than ever, those with arthritis need to understand the benefits and possible side effects associated with all arthritis pain medications. In order to do so, people with arthritis, their caregivers and families must be familiar with recent news about the two types of drugs most commonly used to treat arthritis pain – non-selective, non-steroidal anti-inflammatory drugs (NSAIDs), and another group of NSAIDs known as cyclooxygenase-2 (COX-2) specific inhibitors.

    COX-2 specific inhibitors vs. Other NSAIDs

    COX-2 specific inhibitors are the newest members of the NSAID class of medications. Available by prescription only, they became widely used in recent years to reduce joint pain and swelling. COX-2 specific inhibitors work by selectively blocking, or inhibiting, one of the two enzymes associated with inflammation. Some experts think that this selective inhibition may be one reason for some of the negative side effects currently associated with COX-2 specific inhibitors.

    Non-selective NSAIDs were developed earlier than COX-2 specific inhibitors and have been widely used to relieve arthritis pain and inflammation for many years. Unlike COX-2 specific inhibitors, non-selective NSAIDs inhibit both major enzymes involved in the inflammatory process, COX-1 and COX-2. The non-selective NSAID category includes a number of different medications that are available in both prescription and over-the-counter (OTC) products.

    Timeline of Events

    To understand the current state of affairs, it is important to understand the sequence of events. The controversy started when a study published in the October 21, 2004, issue of the New England Journal of Medicine cited the COX-2 specific inhibitor, Vioxx as potentially causing “major adverse events,” including heart attack and stroke, among patients taking the drug. As a result, Merck (the drug’s manufacturer) voluntarily withdrew Vioxx from the market. However, in the months following, the safety of the other available COX-2 specific inhibitors such as Celebrex and Bextra, as well as other arthritis pain medications in the non-steroidal anti-inflammatory (NSAID) class, were also called into question.

    Consequently, in February 2005, the US Food and Drug Administration (FDA) convened a special Advisory Committee, made up of members of the Arthritis and Drug Safety Advisory Committees, to review the cardiovascular safety of these arthritis pain medications.

    FDA Directive: Stronger Warning Labels for Some Pain Medications

    On April 7, 2005, taking into account the recommendations of the Advisory Committee, the FDA issued the following directives:

    • Bextra, a COX-2 specific inhibitor manufactured by Pfizer, was withdrawn from the market.

    • All prescription NSAIDs must revise their labeling to include a “black box” warning that highlights the potential increased risk for cardiovascular (CV) events as well as the potentially life threatening gastrointestinal (GI) bleeding associated with these drugs. Celebrex, the only COX-2 specific inhibitor remaining on the US market, was included in this directive.

    • All OTC NSAIDs (except aspirin) will be required to revise their labeling to include more specific information about the potential for GI and CV side effects, a stronger reminder to follow label instructions, as well as a warning about potential skin reactions.

    To further evaluate the potential for increased CV risk, the FDA also announced that all NSAIDs must conduct and submit to the FDA a comprehensive review and analysis of pertinent safety data from clinical trials.

    • Aleve (naproxen sodium) is supported by clinical studies conducted to gain approval of naproxen as a prescription product and as OTC that do not provide any evidence of increased risk of cardiovascular events.


    Treatment Options: What Should Arthritis Patients Know?

    For some people who suffer from pain associated with arthritis, their symptoms can be managed with exercise, heat/cold therapy, joint protection, assistive devices, weight control, or in some severe cases, surgery. For others, medications are needed to help manage the symptoms associated with arthritis.

    Smoking is Hard on The Heart

    Atherosclerosis is a disease in which fatty substances such as cholesterol, cellular waste, calcium and other substances are deposited along the lining of artery walls in a person's body. These sticky, yellowish deposits, known as plaque, build up over time, hindering blood flow.

    Also known as hardening of the arteries, atherosclerosis often starts early in life and progresses slowly as a person ages. Atherosclerosis usually affects medium and large arteries in the body. Many scientists believe that damage to the innermost layer of the artery, known as the endothelium is where atherosclerosis begins.

    Damage to the endothelium allows plaque to build up along the lining of arterial walls, and as it does, blood flow is contricted and the supply of oxygen to the body is decreased. Plaque can rupture and cause blood clots (thrombus). These blood clots can break away and enter the blood stream, lodging in another part of the body, sometimes completely blocking blood flow (embolus). Fatty embolisms that block blood flow to the heart cause a heart attack. If they block blood flow to the brain, they cause a stroke. If blood flow to the arms and legs is reduced, it can cause difficulty walking and eventually lead to gangrene.

    Three proven causes of atherosclerosis are:

    Elevated Cholesterol and Triglyceride Levels

    Elevated levels of cholesterol and triglycerides in the blood cause damage to the endothelium. Some cholesterol is necessary, and the body usually produces most of what it needs in the liver. The other source of cholesterol comes from animal fat, and is known as LDL or "bad" cholesterol.

    While our bodies need LDL cholesterol, too much of it can raise cholesterol levels dangerously and put a person at risk for atherosclerosis / heart attack. Foods that come from animals, i.e. chicken, eggs, dairy products, beef and pork contain cholesterol. Foods from plants do not contain cholesterol.

    High Blood Pressure

    Blood pressure is the result of two forces. One is the pressure created by the heart pumping blood through the circulatory system. The other is the force of the resistance of the arteries as the blood flows through them.

    When the heart pumps, it pushes blood through the larger arteries and on into the smaller blood vessels, called arterioles. The arterioles can constrict or expand, and when they do, the resistance of the blood flow is affected. The more difficult it is for the blood to flow, the higher blood pressure will be.

    When high blood pressure goes untreated for a long time and the heart is forced to pump harder to get the blood to flow, the result is often an enlarged and weakened heart muscle. And high blood pressure hurts the arteries and arterioles over time as well. They become scarred and hardened, putting a person at risk for atherosclerosis.

    I didn’t make this one up. Richard Salzman, attorney from Hollywood, Florida wrote a not so humorless article earlier this month which states various health conditions that cause behaviors that can be perceived by police officers as public intoxication. These can get you pulled over and arrested for DUI.

    Salzman writes that motorists in less than perfect health can find themselves wrongly arrested on criminal DUI charges. Since Pre-mentstrual Syndrome (PMS) or Pre-menstrual Dysphoric Disorder (PMDD) is a medical condition that can alter behavior, induce rapid emotional changes, dizziness, irritablity, anxiety and nervousness - these are symptoms that police officers associate with alcolhol or drug impairment.

    In addition to PMS, I also treat many patients with migraine headaches and depression, both of which can lead to altered mental states and confuse arresting officers. For example, sufferers of migraines often have severe dizziness, nausea and vomiting, in addition to intense photophobia rendering them nearly blind in the daylight. Sudden onset of a migraine while under these conditions can lead to erratic driving, poor vision and someone who appears to be quite intoxicated to a police officer once pulled over.

    Depressed patients have overlapping symptoms with someone who is in the lethargic phase of alcohol consumption.

    Treatments for Thyroid Diseases and Conditions

    Recommended treatment approaches depend on the type of thyroid disease you have, and in some cases, the severity of the condition. This article provides an overview of the treatment approaches for key thyroid conditions, as well as information on the specific treatments.

    HYPOTHYROIDISM/HASHIMOTO'S DISEASE

    Hypothyroidism is a condition where the thyroid is underactive, chemically destroyed, or surgically removed, and therefore unable to produce sufficient levels of thyroid hormone. Hypothyroidism is treated by replacing the missing hormone, a hormone that is essential to the body’s key functions. This is accomplished by taking thyroid hormone replacement drugs prescribed by a physician.Some practitioners believe that Hashimoto’s requires no treatment, and will only treat a patient with thyroid hormone replacement drugs when the Hashimoto’s has resulted in hypothyroidism that can be verified through laboratory testing.

    Some practitioners believe that Hashimoto’s, which can be confirmed by testing for thyroid antibodies, warrants treatment in some patients. There is some evidence that treating Hashimoto’s disease with thyroid hormone replacement drugs before the thyroid stimulating hormone (TSH) level rises may alleviate some symptoms. Some research suggests that treating someone with Hashimoto’s but a normal TSH may help prevent elevation of the TSH level and progression to full hypothyroidism. This is discussed further in Treating Hashimoto's When the TSH is Normal.

    On the integrative medicine front, some holistic practitioners recommend iodine supplementation, other nutritional supplements, dietary changes, particular yoga poses, mind-body medicine, and other complementary approaches to help the thyroid.

    HYPERTHYROIDISM / GRAVES’ DISEASE

    Hyperthyroidism is a condition where the thyroid gland is overactive, and produces an excess of thyroid hormone. The gland becomes overactive primarily due to autoimmune Graves’ disease, or in some cases due to thyroid nodules that produce excess thyroid hormone, or viral illness.

    Hyperthyroidism is treated by reducing the excess hormone levels, which is accomplished in three different ways:

    • antithyroid drugs
    • radioactive iodine treatment (RAI), also known as radioiodine ablation
    • surgical removal of all or part of the thyroid, known as thyroidectomy
    • block/replace therapy (BRT)
    • thyroid arterial embolization
    The approach used for treatment depends on the severity of the condition, whether or not the patient is a child or a pregnant woman, and in some cases, the preference or perspective of the treating physician. There are also differences in approaches between the United States and other countries in terms of the treatments used. In the United States, surgery is rarely done for hyperthyroidism (except for pregnant women), and RAI is the treatment of choice, to the extent that some patients may not even be informed that antithyroid drugs or surgery are options. In Europe, however, antithyroid drug therapy is likely to be a practitioner’s first choice for treatment.

    Interest is also growing in thyroid arterial embolization, a new approach for Graves' disease treatment.

    In some cases, key hyperthyroidism symptoms such as rapid heart rate or elevated blood pressure are also treated with drugs known as beta-blockers.

    Most thyroid patients who receive RAI treatment or have surgery eventually end up hypothyroid, and are treated with thyroid hormone replacement drugs.

    Some integrative practitioners recommend stress reduction programs, antithyroid dietary and nutritional changes, traditional Chinese medicine, and other holistic approaches to help an overactive thyroid.

    THYROID STORM

    Thyroid storm is a rare but potentially life-threatening complication of hyperthyroidism. When this condition is suspected, immediate treatment needs to be obtained at an emergency room.

    THYROIDITIS

    Thyroiditis refers to an inflammation of the thyroid. While autoimmune (Hashimoto’s) thyroiditis is common, there are other forms of thyroiditis, including post-partum thyroiditis, De Quervain's (also called subacute or granulomatous) thyroiditis, and viral thyroiditis, among others.

    As noted, in some cases, Hashimoto’s thyroiditis is treated with thyroid hormone replacement drugs. For cases of thyroiditis that are painful, doctors typically recommend a pain-reliever with anti-inflammatory properties, such as aspirin, ibuprofen (Advil/Motrin), or naproxen (Aleve).

    If the thyroiditis is especially acute, doctors occasionally recommend steroid drugs to reduce inflammation, along with thyroid hormone replacement drugs, to allow the thyroid to rest from its job of hormone production.

    Holistic practitioner Stephen Langer, M.D., has shared his recommendations regarding nutritional supplements helpful in treating thyroiditis.

    On the nutritional front, there is some evidence that supplementing with the mineral selenium may help thyroiditis.

    GOITER

    A goiter is the term used for an enlarged thyroid. Goiter can develop in both hypothyroidism and hyperthyroidism.

    A goiter can cause a variety of symptoms, including a feeling of fullness in the neck, pain, and less commonly, may wrap around the trachea, making it difficult to swallow or even breathe.

    Doctors may decide to monitor -- not treat -- a small goiter that is not causing symptoms.

    For a small, symptomatic goiter, doctors typically treat with thyroid hormone replacement drugs. The drugs can slow down or stop the growth of the goiter in some cases, but they don’t typically shrink the goiter.

    If the goiter is large, continues to grow while on thyroid hormone, is cosmetically unsightly, or symptoms continue or worsen, doctors may recommend radioactive iodine (RAI) to shrink the gland, or surgery to remove it all or in part.

    In some cases, goiter can be due to iodine deficiency, in which case, doctors may recommend use of iodine supplementation.

    THYROID NODULES

    Nodules can be malignant (cancerous), benign, or indeterminate. The screening process, which involves blood tests, imaging tests, and typically, helps to identify whether a nodule is cancerous. When the biopsy is indeterminate, many patients choose to get a second opinion on thyroid nodules.

    The thyroid is almost always surgically removed all or in part when cancerous or indeterminate nodules are detected. In the case of malignancy, thyroid cancer treatment protocols are followed, based on the type of cancer detected.

    For small, benign nodules that cause no symptoms, many practitioners prefer to monitor patients. A substantial percentage of the population – some experts suggest the majority of people – have these asymptomatic nodules, and require no further treatment.

    For symptomatic benign nodules, the first course of treatment is frequently the thyroid hormone replacement drug levothyroxine, which can sometimes shrink the nodule, prevent nodules from growing, and help prevent more nodules from forming. Because the treatment is fully effective in only a minority of patients, levothyroxine therapy is considered somewhat controversial.

    If the benign nodule is larger and unresponsive to drug or radioactive iodine treatment, surgery is often recommended.

    Another approach being used for nodules is Percutaneous Ethanol Injection (PEI), where the nodule is shrunk by an injection.

    Another promising new non-surgical technology for nodules is high-intensity ultrasound beam technology.

    Detailed guidelines on treatment for nodules are featured in the American Association of Clinical Endocrinologists’ Medical Guidelines for Clinical Practice for the Diagnosis and Management of Thyroid Nodules.

    THYROID CANCER

    There are four different types of thyroid cancer: papillary, follicular, medullary, and anaplastic. The treatments are based on the type of cancer, and in some cases, the staging -- how far the cancer has spread to other parts of the body -- as well as if it’s a new cancer or a recurrent cancer.

    The treatments for thyroid cancer include:

    • Surgery to remove the cancer, known as a thyroidectomy. Some patients receive a lobectomy, which removes only one side of the thyroid. A near-total thyroidectomy removes almost all of the gland. The entire thyroid is removed in a total thyroidectomy. Lymph node dissection is also sometimes performed during thyroid surgery, to assess the spread of thyroid cancer.

    • Radiation therapy to kill cancer cells. Radiation can be delivered outside the body (known as external beam radiation) or by radioactive iodine (RAI), which is usually administered as a liquid or pill. The thyroid absorbs iodine, and the radioactive form of iodine collects in thyroid tissue and kills cancer cells.

    • Hormone therapy, using thyroid hormone replacement drugs. Administering thyroid hormone, usually at suppressive doses to keep TSH low or undetectable, helps prevent thyroid cancer from recurring, by suppressing the activity of any residual thyroid tissue.

    • Chemotherapy, which uses drugs to kill cancer cells. It may be taken in pill form, or intravenously.
    Early stage papillary and follicular thyroid cancer is treated by surgery to remove all or part of the thyroid, followed by hormone therapy and, in some cases, radioactive iodine. If the papillary or follicular cancer is later stage, lymph node dissection, or external-beam radiation therapy may be added to the treatment. In late stage papillary or follicular thyroid cancer, clinical trials of specialized chemotherapy drugs may also be recommended.

    Treatment for medullary thyroid cancer typically includes total thyroidectomy, and in some cases, lymph nodes are also removed. In the later stages of medullary thyroid cancer, radiation therapy to shrink tumors is used to help relieve symptoms and improve quality of life. In late stage medullary cancer, chemotherapy may be recommended to help relieve symptoms.

    Treatment for anaplastic thyroid cancer can include the following: tracheostomy surgery to create an opening in the windpipe (done for tumors that interfere with breathing); total thyroidectomy to reduce symptoms; external-beam radiation therapy; chemotherapy; and clinical trials studying new therapies.

    Some new therapies for thyroid cancer are currently in investigational or trial status, including the drug axitinib, combretastatin, and lenalidomide.

    Detailed guidelines on treatment of thyroid cancer are featured in the Thyroid Cancer Guidelines from the American Thyroid Association (2006).

    Build Your Own Sauna - What You Need To Know First

    It can be easier than you think to build your own sauna. With some basic carpentry skills and a little specialized understanding you can be relaxing in your own home sauna in no time at all.

    There are a few important decisions to be made to be sure you end up with the sauna that best fits your situation. One consideration is how much work you want to put into the building of your sauna. There are prebuild units that are delivered to your door and literally take only a few minutes to set up. You can also get precut kits that take longer to construct but give you a more custom sauna when finished. The ultimate is to build your own sauna from scratch. Even this method is not as difficult as you might expect. The costs tend to go down considerably as you add more and more of your own elbow grease.

    Another concern is the location. Should your sauna be inside or out, attached or freestanding? This may depend on whether you are in a rural or city environment. Locating near a water source is also important. If you are outside this can be a lake or pond to jump into. For indoor saunas a nearby shower can be used to rinse off. In either case before you start building think about getting water to your sauna. Along with a convenient way to rinse off it is nice to have an area to change into and out of sauna apparel. Benches for sitting on while removing clothes and hooks for hanging them are a great feature.

    Deciding how you will heat your sauna is a major decision. Conventional saunas are heated with either wood, electric or gas heaters. Any of these can be used dry or wet depending on whether you sprinkle water on the heated rocks or not. Some people like it dry and some prefer wet, it comes down to personal tastes. Before deciding which kind of stove you want think about getting fuel to your heater. Wood may be difficult to acquire in the city but electric or gas hookups may be impractical in more remote areas. Costs can vary greatly depending on the brand and type of stove you get. A relatively new (compared to the 2000+ year history of saunas) type of sauna is the infrared sauna. These use infrared emitting lights to penetrate and heat your skin. It is a little like your microwave oven in that the food (you) is heated but the surrounding air is not. Infrared saunas are always dry. Because of the complexity of installing and correctly positioning the infrared wave emitters I suggest you buy a prebuilt model if you go with infrared.

    There are a few special concerns that distinguish building a sauna from other conventional frame construction. Moisture is a concern. There should be a vapor barrier to keep the heat and steam from escaping. Any electrical connections need to be protected from excessive moisture. Ventilation is necessary to keep the oxygen level up and if done right it will also help to evenly distribute the heat.

    All in all building a sauna is not an overwhelming task. If you can do some basic framing and simple finished woodworking you are well on your way to building your own sauna that can be enjoyed for years to come.

    Guide to Breast Augmentation: Part one

    Sydney plastic surgeon Dr Kourosh Tavakoli explains the key elements to a natural-looking breast augmentation.

    The number of breast enlargement procedures is on the increase, with a trend toward natural-looking breast implants. A well-augmented breast has a natural fullness, with gentle sloping off the chest wall. There should be natural cleavage without webbing between the breasts and only a certain amount of perkiness.

    There are three main telltale signs of unnatural breast augmentation:

    1. Breast implant margin This should be soft and imperceptible, as obvious implant edge visibility, especially in the midline, will resemble the 'Tori Spelling' look.

    2. Breast implant size An implant that is too large for a small frame is usually an obvious giveaway that a patient has had a breast augmentation. The best example of this would be 'Pamela Anderson' breasts.

    3. Perkiness An augmented breast that is too perky will tend to look fake, as a small amount of droop is natural. When the patient lies on their back the breast implants should roll to the side like natural breast tissue and not sit up like rigid peaks.

    Before undergoing surgery, it is beneficial for the patient to review photographic examples of the breasts they would like, and to convey their wants to the surgeon so there is a clear, visual understanding of the desired result.

    There are six main clinical parameters for breast implants on initial consultation:

    1. Patient's body build and height
    2. Natural breast size, shape and symmetry
    3. Nipple position in relation to the breast
    4. The quality of breast skin
    5. Chest wall shape and dimension,
    6. Patient's desire for cup size and shape.
    After establishing the crucial parameters in the clinical examination, the process of tailor-making the 'right' breast augmentation begins.

    Six key considerations

    I believe there are six key considerations for a surgeon when customising a breast augmentation to gain a naturallooking result. I discuss three of these key considerations below and the final three in the next issue.

    1. Incision placement

    There are three choices about where to make the skin incisions for breast enlargement. They can be in the breast fold (inframammary), around the nipple (periareolar) or underneath the arm (transaxillary). These incisions can all produce scarring. Although patients may voice some initial concerns about the location of their scars, they are ultimately far more with the final shape and size of their breasts.

    Generally, a great majority of patients in my practice opt for the inframammary incision (breast fold). I also find this incision has the least amount of interference with breast feeding and nipple sensation, and it generally heals very well.

    2. Breast implant shape

    The choice of implant shape varies from round to teardrop. The round implant comes in both low- and high-profile varieties. The majority of patients in my practice elect to have round-shaped implants. Since I prefer placing the implant in a submuscular pocket in most patients, implant edge visibility in the upper pole of the breast can be avoided. The round implant tends to be ideal for those patients with good-shaped natural breasts who desire a straightforward enlargement.

    Use of the teardrop (anatomical) shape depends on the patient's desire, as well as her body shape. The shape variation is in the width and projection of the implant for any given size. The spectrum of breast implants available to the surgeon can therefore provide great versatility in achieving a more natural look.

    In general, there are two groups of women who benefit from teardrop-shaped breast implants. It can be the ideal choice for women who have droopy or tuberous breasts. Mild elevation of the nipple in relation to the breast mound can be achieved without the need for extra scars on the breast (unlike a breast lift). In these cases, the implants will be inserted in a subglandular (subfascial) pocket (under the breast tissue).

    Secondly, some patients specifically want less fullness in the upper quadrant. Teardrop breast implants certainly offer less fullness in these particular situations. It should be noted, however, that this request tends to be very personal, as most women are seeking breast augmentations in order to obtain upper pole fullness.

    Teardrop implants do have a slight tendency to rotate. Unfortunately, this problem can only be corrected by secondary surgery.

    3. Breast implant fill

    I use both the cohesive silicone-gel and saline-filled breast implants. The new generation silicone-gel breast implant is very safe and generally feels and looks more like a natural breast. Most breast augmentation operations in Australia are performed with silicone-gel implants. In December 2006, the Food and drug Administration (FDA) approved the use of gel implants in the US market. The decision was based on a great amount of scientific research into siliconegel implants.

    Cycling is a great way to stay fit, but it can also be the cause of pain and discomfort.

    Cyclists sometime acquire a type of knee pain known as IT band syndrome. This painful condition is caused by a stretching of the iliotibial tissue. This thick, fibrous tissue that runs form the hip to the knee becomes stretched, due to the constant up and down motion of the legs while cycling. When stretched, the tissues begin to rub against the bony portions of the knees or hip, causing serious pain.

    Beginning cyclists commonly complain of IT band syndrome pain. These rookie riders can cycle for hours without giving due respect to the needs of their feet and knees. Other causes of knee pain are poor sitting position, cleat alignment, hard riding or a faulty saddle. It can even be a result of simple body anatomy. The only way to reduce this pain is to ride more slowly. This allows the body more time to adapt to the physical stress of cycling. Minimizing hard riding and stretching before and after riding can also help lessen the effects.

    There are changes that you can make to your bicycle to help reduce cyclists' knee pain. Try adjusting the seat higher or lower to take stress off of the knees. Whether your seat is too high or too low, you can experience cyclists' knee pain. If the seat is too high, you'll feel the pain behind the knee. On the other hand, a seat positioned too low can result in pain in the patella. Visit your cycle shop for help in finding the best seat height position for you. If your bicycle is equipped with cleats, they too may need some adjustment. Cleats should not float too much internally, and should not float externally. Ideally, the cleats will be neutral. Your bike shop can help to adjust your cleats using the "fit kit" method.

    Another common cycling-related injury is chondromalacia. This painful condition is caused when the cartilage behind the patella is under constant pressure, such as in cycling.

    Try strengthening the quadriceps muscles along the front of the thigh to relieve your discomfort. Some doctors recommend simple exercises, like sitting on a chair and stretching the leg, to strengthen the quadriceps. Avoid sideward movement while pedaling to help ease your pain.

    If cyclists' knee pain strikes you, apply ice packs or rest your knee. You'll also need to reduce your riding time until the knee pain subsides, or risk further damage. Try to avoid cycling across bumpy surfaces or riding at high speeds while your knee is on the mend. Try stretching and icing before and after the ride to help keep your muscles loose. It's also important to keep your legs warm during cold weather riding.

    If you suffer from persistent pain, or the condition becomes worse, see your doctor. Cyclists' pain can indicate a more serious condition, so it's a good idea to get a professional opinion before treating the symptoms.

    The bad news about heart disease is that it remains extremely prevalent in our society. The good news is that the factors that determine our risk of developing heart disease are, to a large extent, under our control. We ourselves have a lot to say about whether we will develop early cardiac disease.

    Our doctors are supposed to assess our risk for us, and coach us on what we should be doing to reduce that risk. But, despite pleas from medical experts and professional societies to the contrary, doctors are still generally bad at assessing their patients' risk of developing coronary artery disease, and are particularly terrible about spending the time needed to stress the importance of taking the appropriate steps to reduce that risk.

    (Note well: Doing simple risk assessment is one of the most important basic jobs of the physician. The failure to do such risk assessment probably ought to be considered evidence that your doctor is doing sub-standard work. Many of the things doctors need to think about are complicated - this one isn't. DrRich, for one, wouldn't stick with a doc who isn't even doing the simple things right.)

    Fortunately, the tools exist today for individuals to accurately assess their own risk of getting heart disease, without waiting for their doctors to initiate the action. And lots of information is available as to what to do about it if your risk is elevated.

    To assess your own risk, here's what you need to know

    You need to gather the following information:

    • whether you smoke or not
    • your total and HDL cholesterol levels
    • your blood pressure
    • whether you have evidence of diabetes or metabolic syndrome
    • whether you are overweight for your age and height
    • your family history

    With this information, you can place yourself into one of three categories: low, intermediate, or high.

    With this information, to be in the low risk category, all of the following must be present:

    • nonsmoker
    • total cholesterol <> 40 mg/dL
    • systolic BP <>
    • no evidence of diabetes
    • not overweight
    • no family history of premature vascular disease

    You are in the high risk category if you have any of the following:

    • known coronary artery disease or other vascular disease
    • type 2 diabetes
    • over age 65 with multiple (more than one) risk factors

    And you are in the intermediate risk group if you don't fit into either the low or high risk groups.

    Low risk patientscan be managed without any further intervention, except for routine coaching on maintaining a healthy lifestyle. About 35% of U.S. adults fall into this category.

    High risk patientsshould immediately be placed on appropriate treatments proven to reduce the risk of heart attack and death, such as statin drugs, beta blockers, aspirin, and/or ACE inhibitors.     Intermediate risk patients, in addition to taking steps to modify the risk factors keeping them out of the low risk category, should have non-invasive tests to measure whether or not they already have evidence of coronary artery disease, such as stress/thallium testing or electron beam tomography (EECP).
