Contact Allergies

The symptoms of contact allergies normally occur within a few moments of exposure to the allergen; however, it is possible for reactions to the allergens to show up several hours after exposure. There are many common allergens such as poison oak and poison ivy as well as medications, latex, dyes, fragrances or perfumes and cosmetics, preservatives and nickel, rubber and hair dyes. Nickel is a frequently used metal in jewelry, hairpins, buttons and zippers as well as snaps and metal clips and is frequently associated with allergies. How to Burn More Calories Without Working Out Longer

Time is a limiting factor for many people who workout and want to workout. Even the most good intentioned dedicated gym goers and workout enthusiasts sometimes find themselves in a bind with other commitments and cannot “find the time” to get to the gym. The ever increasing pace of life requires us to be very good at time management to fit everything we want into a day, especially our workouts. When it comes to working out I’ll bet you have a set amount of time you can dedicate to it each week and that’s it, there is no room for increasing that time. This is probably the case for almost everyone that works out on a regular basis. So if we can’t ever increase the time we spend working out, how to do we increase the results we are getting? There are a few things you can do.

In general there are only 2 basic things you can change in each workout to progress and continue to get the benefits you want.

1) Increase the amount of time working out (but as we are discussing here this is not an option for most people)

2) Increase the intensity of each workout. This can be accomplished multiple ways:
a. Increasing the amount of weight you are using
b. Use more muscles with each exercise
c. Do more reps in the same amount of time
d. Decrease the amount of rest you take between exercises

Increasing Weight used:

Even if you aren’t doing traditional weight training, you can increase the amount of work you are doing by adding some weight to any exercise you are doing. For example of you do some interval running and want to increase the amount of work you are doing you can add a weighted vest for some of your run. A vest that adds 5 or 10lbs to your weight will make you work much harder during your run. You will end up burning more calories and improve your conditioning beyond just running with your regular body weight. You wouldn’t have to do your whole run with the vest; maybe just add in 20% of the run with the vest to start.

If you are doing weights in your workout (which I would highly recommend as a regular part of your weekly workout routine) then the easiest way to increase the amount of work you are doing is to increase the weight. This means increasing the weight and doing the same number of reps. This technique works if you are working with weights well below the maximum weight you can handle. If you are already working at or near your maximum weights then you will obviously not be able to increase the weight as by definition of max you would not be able to lift anything heavier. This however is not the case for most people, and there is usually some room to move up. Even if you only go up 5lbs, that is still more work done.

Use More Muscles with Each Exercise

This is a simple and very effective way to get more work done without extending the time. For example if you are doing a bodyweight squat, you can add in a shoulder press at the same time. Now you are working your legs and hips as well as your shoulders and arms. This is substantially more work than squats by themselves. This technique of using more muscles can be applied to almost any exercise you can think of.

More Reps in the same amount of Time

This simply means doing your reps faster. So if you can do 10 bicep curls in 10 seconds. Try to do 15 in 10 seconds (with the same weight). This will be 50% more work done. You can apply this technique to almost any exercise. In the case of running or other cardio type exercises, this would just be taking more steps, or in other words, running faster. I would suggest adding in short bursts of faster running, and more reps. For example if you go for a 30 minute run. Now you could add in 30 second bursts of running faster every 5 minutes. This would increase the work done without increasing your time spent.

Decrease the Rest between Exercises

If you do an intermittent style of workout as many people do, there will be periods of rest between exercise sets. Shorten your rest periods and the amount of work you are doing will increase. This also allows you to fit a few more sets in the same amount of time.

You can apply any of these techniques to increase the amount of work you are doing and the amount of calories you are burning without every working out for any longer. I have designed workouts that incorporate most of these techniques and they produce great results. This is the new are of working out with incredible time constraints so the workout you are doing must be adaptable if you want to continue to progress.

Just How Much Vitamin C Do You Need For Optimum Health?

Many benefits of vitamin C have been identified since the consumption of citrus fruit was first recognised as the immediately effective cure and preventative for the scurvy which so dreadfully afflicted the long distance sailors of a few centuries ago.

Numerous studies have recognised the vitamin as a possible protector against coronary heart disease, stroke, atherosclerosis, hypertension and cancer, to name but a few, and it is possible that all of these effects stem ultimately from vitamin C's acknowledged role as a powerful anti-oxidant, active in preventing the free radical damage which is a known cause of premature ageing and many chronic degenerative diseases.

So the absence of acute deficiency diseases such as scurvy in modern affluent societies should not be taken as implying that the typical Western diet contains an adequate intake of vitamin C for optimum long-term health.

The United States Recommended Dietary Allowance (RDA) for vitamin C is 90 mg per day for a healthy, non-smoking man and 75 mg for a woman. But these suggested requirements are increased to 125mg and 110 mg respectively in the case of smokers, a tacit acknowledgement of the increased requirement for this vitamin which increased toxic stress places on the body. But these RDAs nevertheless appear to be set at a level sufficient only to avoid outright deficiency disease.

To be sure of obtaining all the optimum benefits of vitamin C, the authoritative Linus Pauling Institute has recommended a daily minimum intake of at least 400 mg. This amount has been shown to achieve saturation levels of the vitamin within the body's tissues in healthy young adults who are non-smokers. To get this in perspective, even the consumption of the recommended five daily servings of fruits and vegetables may provide just 200 mg of vitamin C, whilst even commercial multi-vitamin supplements typically provide only around 60 mg.

But the real story's actually even worse than this, not only because all too few people actually manage to consume the recommended quantities of fruit and vegetables, but because the nutritional quality of those that we do consume is poor, and getting worse.

As long ago as 1936, Senate Document 264 noted that the poor quality of American farm soils was leading to widespread nutritional deficiencies, and the 1992 Earth Summit reported that mineral concentrations in US farm soils were 85% lower than those of a hundred years ago.

Nor does the problem lie just in the soil. The modern Western diet's preference for highly refined grains, and the treatment of fruits and vegetables with preservatives, dyes, pesticides and even radiation is a proven disaster for vitamin and mineral retention, as well as a significant toxic assault on the body.

Indeed, the problem for us in the twenty-first century is that our environment seems as though it might have been expressly designed for ill-health. Daily we're exposed to a kind of toxic soup of pollutants such as industrial emissions, car exhausts, pesticides, herbicides, dyes and all kinds of everyday household chemicals. But as the liver works ever harder in an increasingly desperate struggle to detoxify the body, an unwelcome side effect is that it produces enormous quantities of the free radicals which are amongst the chief contributors to premature ageing and degenerative disease.

In these adverse circumstances it can only make sense to ensure that the body is as lavishly provided as possible with the top quality anti-oxidants of which vitamin C is certainly one of the most important.

Fortunately, high dosages of vitamin C are readily available as supplements and fortunately, too, it seems that the manufactured kind of l-ascorbic acid (vitamin C) is chemically identical to that obtained naturally. This is not in any way to deny the importance of healthy eating, or to suggest that it doesn't make sense to try and eat the recommended five daily servings of fruit and vegetables, because these contain a myriad of trace nutrients which operate synergistically within the body, and which all need to be present for optimum health.

It does suggest, however, that to rely on diet alone, however apparently healthy, may well be to risk missing out on the vital anti-oxidant properties of vitamin C which may in time be reflected in chronic, degenerative, if not acute, disease. It should be noted as well, that as a water-soluble compound, any excess vitamin C is harmlessly excreted by the body.

So with all the health benefits of vitamin C at stake, it surely makes sense to err on the side of taking in too much rather than too little.

Nine Anti-aging Tips to Stay Younger Longer

Can't figure out where to start to improve the quality of your lifestyle? Here are some simple things to help get started. If you can follow the suggestions for a month, you will be well on your way to reducing belly fat AND establishing an anti-aging lifestyle.. It's not difficult – if you really want to do it. Remember, these are baby steps, but you have to start some place.

1. Check your fridge. If you use butter, you can use less by diluting it half-and-half with olive oil. Let a bar of butter soften to room temperature. Beat it smooth and gradually beat in an equal amount of olive oil. It will be runny but will set up to a soft spread in the fridge.

2. Rethink the mayo. If you can't live without it, make your own with olive oil. Then you will really have something worthwhile to spread on bread or to make a salad.

3. If you eat peanut butter, buy Smart Balance peanut butter for it's omega- 3 content. If you are not taking an omega-3 supplement, you should.

4. Keep a gallon bottle of water on the kitchen counter or at work. Drink at least half of it every day. Having it in plain sight will help you drink more of it.

5. Check your pantry. Don't restock chips, cookies, soda, toaster pastries, sugary breakfast "cereals" (a misnomer if ever there was one!) It's just as quick and easy to scramble a couple of eggs for breakfast as it is to wait for a pastry to toast. I know how tough it is to get rid of the nutritionally bankrupt junk, so do it in small steps if you can't do it cold turkey.

6. Substitute whole grain bread for white bread. Beware of "wheat" bread – it's probably not whole wheat. If possible, go to a whole foods market and get some real whole wheat bread. In my area, I can buy incredibly good whole grain bread from Alvarado Street Bakery. You can buy it online, and if you do, put it in the freezer the minute it arrives. Because it's "live" and preservative free, it will get moldy very quickly if you leave it out. I particularly enjoy the Alvarado sprouted whole grain bread.

7. Having a whole foods market nearby makes it easy to cut down on processed food. If you make a trip a couple of times a week you can get all the super-fresh veggies and fruit you will need to provide a healthier diet for your family. For protein, focus on chicken – preferably baked or roasted. I use a lot of ground turkey – you can prepare it so many ways. If you work and are pressed for time, many whole foods markets offer excellent take out.

You will notice I did not mention fish. I rarely buy fish because it's so difficult to get good fish. If I can't get wild salmon or other fish that hasn't been farm raised, I don't bother. I would never eat shrimp because it causes too many intestinal problems. There is another way to get the benefits of eating fish – in fish oil capsules. True, you run the risk of contamination there as well. But I think I'd rather opt for the capsules than supermarket fish that looks like it's seen better days, and probably has.

8. Skip fried anything as much as possible. And you certainly won't need your deep fat fryer anymore, so throw it away. (Well okay, give it away. But you won't be doing anyone a favor.)

9. If you are an emotional eater, eat unbuttered popcorn or dry roasted nuts, such as almonds or cashews. You can flavor popcorn with garlic salt or any seasoning of your choice. Or try spritzing on garlic flavored olive oil. Drink green tea to wash it down. Most commercial green teas are bitter or tasteless but I have found some very tasty teas at Clipper teas.

This should be more than enough to help you start to live a healthier, anti-aging lifestyle. Start with at least one step and add others as you develop a system.

The value of proper sleep should also never be underestimated. A good night’s sleep also helps with boosting immune system functioning. In the busy world of long work hours and the demands of home and family it can be difficult to get enough sleep each night. You can feel rundown and drained of energy, which I'm sure we all have certain times of our lives. When a person is lacking enough sleep they become much more susceptible to contracting illness and disease.

Boosting immune system function can be done with just a few steps. Your immune system will respond to a healthier diet, exercise and a reduction in stress. There are absolute ways in which you can help yourself but it must start with you. The responsibility is yours.

Low-carbo, high-protein diets increase cholesterol

Now, anybody who wants to lose weight knows Atkin diets.

Sure, they are said to be effective in losing weight easy and fast, which is exactly what most of us dream of.

But hey, a report in the Archives of Internal Medicine pointed out that low-carbohydrate, high protein diets are found to increase our cholesterol levels. And we are referring to both LDL (low density lipoprotein, which is also known as "bad" cholesterol) and HDL (high density lipoprotein which is also the "good" cholesterol).

HDL helps the body get rid of bad cholesterol. LDL is said to cause build-up of plague on the artery walls, contributing to risks of cardiovascular diseases.

The findings were from an analysis of the five clinical trials comparing low-fat diets with low-carbohydrate diets. These trials attracted 447 participants whose ages range from 42 to 49 years old. Out of 447 participants, 225 are on low-fat diets while the rest are on low-carbo diets.

Both groups were then tested for their weight loss and blood pressure after six and nine months' period. It was expected that for the first 6 months, the participants on the low-carbo diet had lost more weight. After 12 months, the outcome of weight loss and blood pressure turned out to be the same for both groups.

What raises our concern was that from the results of low-carbo diet group, total cholesterol increased throughout the 12 months period. A low-carbo diet is usually high in fat, resulting in a higher level of cholesterol.

However, the results further show that a higher cholesterol level had led to a concurrent increase in both LDL and HDL. They also had a lower level of triglycerides, a type of fat, which LDL carries in the blood.

So does the beneficial effects of low-carbo diet appear to have on HDL and triglyceride levels cancel out their apparent negative effects on overall and LDL cholesterol levels?

Not necessarily so, as another study published in the Journal of Lipid Research in 2003 showed that high triglyceride levels, high LDL levels and low HDL levels are actually independent risk factors for cardiovascular disease.

So despite what the low-carbo diet results show, more evidence is needed to support the conclusion as the study had its limits. The researchers in the low-carbo diet report also expressed concern about the effects of the low-carbo diet on cholesterol levels and the cardiovascular system as it contains large amounts of fats and proteins.

This is because you are more likely to consume more foods high in saturated fats when you turned to high-protein diet. This increases your calories as fat provides more calories per gram than carbohydrates. Fat contains nine calories per gram while carbohydrates provide four calories per gram.

You are putting yourself at a higher risk of contracting coronary heart disease, if you do not reduce your fats intake.

Other health problems associated with a diet high in protein include kidney failure, high cholesterol levels, osteroporosis and kidney stones. As a guide, an adult should obtain 50 to 60 percent of their calories from carbohydrates, 10 to 15 percent from protein and 20 to 30 percent from fats.

Therefore, it is not advisable to go long term on low-carbo, high-protein diets.

Stress-relief: Techniques That Work For You!

Stress can come in many forms - from workplace tension, marital woes, family squabbles, financial perils - only to name a few. Stress triggers your body's "fight or flight" response either enabling you to flee from harm (as in escaping a potentially harmful situation) or, more likely in our modern times, it triggers a response to a given situation or event, such as an argument, traffic jams, deadlines, etc. These situations increase our blood pressure and breathing as if we were to flee.

Chronic stress keeps our body in this "fight or flight" reaction thus leaving it at an unhealthy level that negatively affects our immune system. In fact, studies have shown that stress is related to 90% of all illnesses.

How do you recognize stress? Stress has both physical and emotional factors with each presenting their own list of symptoms, such as an inability to sleep, headaches, pain in back, shoulder, or neck areas, trouble thinking clearly, lack of concentration, and memory loss.

Learning to handle stress is essential to good health. Long-term stressful situations lend themselves to obesity, heart disease, diabetes, cancer, tooth and gum disease, hyper-thyroidism, and other major health issues.

What techniques can you use to help you to cope with stress?

* Have a distraction free bedroom by removing the TV and all work-related items from the room.

* Avoid staying in your bedroom when not sleeping or relaxing; use only at bedtime.

* Use relaxation exercises to learn how to relax, such as the one below, even while at work during a break or lunch.

* Eat right and exercise regularly, even if it is only 30 minutes a day three or more times a week.

* Plan your daily activities according to priorities. Create a check list.

* Mentally or physically check off completed tasks which will give you a sense of accomplishment.

* Participate in wellness programs or create one of your own by receiving a weekly massage, pedicure, body wrap, or another relaxing treatment.

If your stress is due to a major crisis as in disasters caused through nature, war, or an act of violence, professional help may be needed to learn to cope with the multiple emotional and physical issues involved. Consult with your health care provider about your stress factors.

Relaxation Exercise that you can use anywhere:

*Close your eyes.

*Take a deep breath.

*Inhale and exhale slowly throughout this whole process.

*Concentrate on your toes.

*Will your toes to relax.

*Next concentrate on the bottom of your feet.

*Relax the bottom of your feet.

*Continue until you work your way up to the top of your head.

What Doctors Don’t Tell You about Your Allergic Rhinitis

As a medical student I have access to the same clinical reference databases your doctor uses. One of the most widely used databases, called UpToDate, reviews all the pertinent literature and then develops treatment guidelines for various diseases and conditions.

As I was perusing the Allergic Rhinitis section, it struck me that there was some important information in here that doctors probably weren’t telling their patients. I believe it’s important for everyone to be fully informed about their own health so I’m going to go ahead and share this information with you.

1. If you must use nasal decongestants make sure you only use them for a week at the very most:

Nasal decongestant sprays are not recommended in the treatment of allergic rhinitis. Tachyphylaxis (rapidly decreasing response to a drug following administration of the initial doses) develops after only three to seven days. Then rebound nasal congestion results, and if you continue to use the nasal spray anyways it causes rhinitis medicamentosa. Which is literally rebound nasal congestion caused by decongestants. Even worse, the swelling of the nasal passages may eventually result in polyps that need to be surgically removed.

2. Use a saline nasal spray or wash before your topical nasal medications:

Topical nasal corticosteroids are now the first line of treatment for allergic rhinitis. Intranasal steroids are more effective than cromolyn and second generation antihistamines in improving the symptoms of allergic rhinitis and the symptoms of seasonal asthma. BUT treatment failures may occur if mucus or other debris is not cleaned from the nose before application. Saline nasal sprays or washes should be used to clean the nasal passages before you use your meds.

3. Save your money! You might not be a good candidate for allergy shots:

A large number of controlled studies have demonstrated that allergen immunotherapy to tree, grass and ragweed pollens, mold and house dust mite are effective in the treatment of allergic rhinitis. But if you are on beta-blockers (often used in the treatment of high blood pressure and heart problems) you aren’t a candidate for allergy shots.

If you aren’t on beta-blockers and are willing to try a 2 year minimum (normally 3-5 years) trial of allergen immunotherapy then consider this as well. Individuals who don’t have easily demonstrable quantities of allergen-specific IgE (antibodies) by prick testing are not candidates for allergy shots. So you may save yourself a great deal of time and money by making sure your doc checks your IgE levels first.

If you decide against the allergy shots, but aren’t enamored with the thought of taking symptom-suppressing medications for the rest of your life then you might try some alternative treatments for allergic rhinitis. Homeopathy is an alternative field of healthcare which focuses on restoring the health of the individual rather than suppressing the symptoms of their disease.

In particular, an encouraging study was done on individuals living in the southwest who suffered from seasonal allergic rhinitis. The participants were given a homeopathic regional allergy mix (a medicine prepared from allergens specific to the southwest that in its final form is incredibly dilute). The study showed significant reduction of symptoms and improvement in quality of life over a 4 week period in the homeopathic group compared with the placebo group.

There are homeopathic medicines available that contain the appropriate allergen complex for any area of the U.S. The allergen complex most often contain specific grass, trees, weeds, pollen and mold that are found in high concentrations in specific regions. These regions include:

Southern U.S.- AR, LA, MS, AL, GA, FL, TN, KY, WV, VA, NC, SC, & east TX

Rocky Mountains- CO, ID, MT, UT, WY, & NV

Southwestern U.S.- OK & TX

Desert- UT, CO, CA, & NV

Northeastern U.S.- ME, NH, VT, MA, CT, RI, NY, NJ, PA, DE, MD

Plains- ND, SD, NE, & KS

Pacific- WA, OR, & CA

Great Lakes- MN, WI, MI, IA, MO, IL, IN, & OH

Don’t despair if your allergic rhinitis is caused by ,other than those mentioned above. There are also homeopathic formulations which contain allergen complexes such as animal hair & dander, and dust, mite & roach. These 2 causes of allergic rhinitis have higher remission rates than pollen allergy and may respond even more favorably to a homeopathic attempt at allergy correction.

Thyroid Surgery and Thyroidectomy

The following article looks at the various issues surrounding thyroid surgery, known as thyroidectomy.

Reasons for Thyroid Surgery

Thyroid surgery is performed in a number of circumstances:
  • as a treatment for thyroid cancer
  • when an enlarged thyroid (goiter) or multiple nodules cause cosmetic, breathing or swallowing problems
  • in a pregnant woman, when her hyperthyroidism is not controllable by antithyroid drugs, and requires immediate treatment
  • when other forms of treatment for hyperthyroidism -- i.e,. antithyroid drugs or radioactive iodine have not been effective. (This is applicable in the U.S. Outside the U.S., surgery is sometimes performed as a hyperthyroidism treatment before or instead of radioactive iodine.)
  • in children, if the practitioner or parent wishes to avoid radioactive iodine
  • when the patient refuses antithyroid medications or radioactive iodine
  • when a patient wants to try to get pregnant quickly after treatment

Choosing a Surgeon

Complications are more likely with surgeons who have less experience performing thyroid surgery, so make sure that your surgeon has extensive experience in thyroid surgery. You can find out more in the article about Finding a Top Thyroid Surgeon.

Types of Thyroid Surgery

There are three main types of thyroid surgery:
  • Total Thyroidectomy -- Complete Removal of the Thyroid
    This is the most common type of thyroid surgery, and is often used for thyroid cancer, and in particular, aggressive cancers, such as medullary or anaplastic thyroid cancer. It is also used for goiter and Graves'/hyperthyroidism treatment.
  • Subtotal/Partial Thyroidectomy -- Removal of Half of the Thyroid Gland
    For this operation, cancer must be small and non-aggressive -- follicular or papillary -- and contained to one side of the gland. When a subtotal or partial thyroidectomy is performed, typically, surgeons perform a bilateral subtotal thyroidectomy which leaves from 1 to 5 grams on each side/lobe of the thyroid. A Harley Dunhill procedure is also popular, in which there's a total lobectomy on one side, and a subtotal on the other, leaving 4 to 5 grams of thyroid tissue remaining.
  • Thyroid Lobectomy -- Removal of Only About a Quarter of the Gland
    This is less commonly used for thyroid cancer, as the cancerous cells must be small and non-aggressive.
The issue of a subtotal/partial, vs. total thyroidectomy is controversial. Some practitioners prefer to perform a partial thyroidectomy whenever possible, believing that they will leave behind enough thyroid tissue to prevent hypothyroidism. (A total thyroidectomy has nearly a 100 percent chance of causing hypothyroidism). The risk of hypothyroidism with subtotal thyroidectomy is, however, quite high, and some experts say that more than 70 percent of patients receiving a subtotal thyroidectomy will become hypothyroid. Since one of the main reasons for subtotal thyroidectomy is to prevent hypothyroidism, and that goal is achieved in only a minority of cases, experts increasingly believe that there is no added benefit to subtotal thyroidectomy, and are more routinely recommending a total thyroidectomy.

What You Are Likely to Experience

In most cases, surgery of the thyroid is not highly complicated, and usually takes no more than two hours.

Removal of half of the thyroid takes 45 minutes to an hour, so if the entire gland is being removed, the surgery will last about an hour and a half.

Check with your surgeon about medications you are taking, and what you should/shouldn't take in the days prior to surgery.

You will most likely be asked to check into the hospital the morning of your surgery. Typically, your surgeon will ask that you refrain from eating or drinking after midnight the night before surgery.

Outpatient or Hospital Admission?

Depending on the condition of the patient, an overnight or two-night hospital stay may be planned. Outpatient thyroid surgery is becoming increasingly popular, however, and research shows that outpatient thyroid surgery is safe, effective -- and less expensive -- for most patients, and may be preferable to traditional inpatient hospital stays.

General or Local Anesthesia?

Thyroid surgery is more commonly performed with general anesthesia. Some surgeons are now using local anesthesia, plus a sedative, however, to perform thyroid surgery.

The benefits of local anesthesia are that it is associated with a shorter hospital stay, shorter actual surgery time, and less vomiting and nausea after surgery.

If you choose local anesthesia, your doctor will typically give you numbing medication for the thyroid area, plus a mild sedative to help you stay calm. You will, however, be awake during the surgery, and able to interact with your surgeon.

Not many surgeons are trained to do thyroid surgery under local anesthesia. So if you want to proceed with this option, be sure your surgeon has done a number of thyroid surgeries with local anesthesia. (Some experts suggest you look for a surgeon who has performed this procedure at least 50 times.)

The Surgical Procedure

In the surgery, the surgeon will cut a 3- to 5-inch incision across the base of your neck in front. The skin and muscle are pulled back to expose the thyroid gland. The incision is usually made so that it falls in the fold of the skin in your neck, making it less noticeable.

Blood supply to the gland is "tied off," and the parathyroid glands are identified (so that they can be protected). The surgeon then separates the trachea from the thyroid, and removes all or part of the gland.

An In-Depth Look

A newer technique, known as endoscopic thyroid surgery, involves using a small magnifying camera inserted in the neck. Carbon dioxide gas is pumped into the neck area to help make it easier to see and work on the gland. A second small incision is made, and a thin tube with a scalpel-like edge is inserted through that incision. This tube is the surgical tool that is used to remove the thyroid. Endoscopic surgery, because it involves two small scars of less than one inch, usually leaves less visible scarring, and allows a quicker return to normal activity. Sometimes, the entry point is under the arm -- known as axillary surgery.

Endoscopic surgery is not common, however, and you'll need to investigate to find a surgeon with experience doing these surgeries.

Most surgeons use dissolvable stitches, but you may want to ask your surgeon ahead of time which kind he plans to use, because the non-absorbable stitches actually tend to cause less scarring. If you have any history of allergic skin reactions to past stitches, you may also want to ask your doctor about using hypoallergenic suture material.

After the surgery, you will usually remain under observation at the hospital for at least 6 hours. If you are having outpatient surgery, you may be discharged after that point.

Before you are discharged, your incision is usually covered with a clear protective waterproof glue called colloidium. (This allows you to bathe or shower after the surgery.)

Rarely, if there is concern about bleeding or if the thyroid is very large and the surgery has left a large open space, a drain will be left in your wound to prevent fluid from accumulating. You'll need to return to the surgeon a few days later to have the drain removed.

After Your Surgery

Thyroid surgery is generally considered extremely safe. There are some common short-term side effects after thyroid surgery such as pain when swallowing, and neck stiffness. Most patients also become hypothyroid after surgery and require thyroid hormone replacement therapy. While complications are not common, a few can result from thyroid surgery. These include hypoparathyroidism and hypocalcemia, and laryngeal nerve damage. Signs can include numbness and tingling around your lips, hands, and the bottom of your feet, muscle cramps and spasms, bad headaches, anxiety, depression, hoarseness, and difficulty speaking loudly. Heart Disease Prevention | The Diabetes-Heart Disease Connection

If you saw a tiger, you'd run for your life. But what if you didn't see it? For those of you with type 1 or type 2 diabetes, heart disease is that unseen tiger. You're two to four times more likely to develop a heart condition than people without diabetes? Even more shocking, those individuals are more likely to die from heart disease or other cardiovascular ills than from the complications of diabetes itself. Yet surveys show that 68 percent of Americans with diabetes are unaware of their increased cardiovascular risk.

If you have diabetes, now is the time to take steps to protect your heart.

"High blood glucose alters cell metabolism," says Richard Kahn, Ph.D., chief scientific and medical officer of the American Diabetes Association (ADA). When the function of platelet cells changes, the likelihood of blood clots increases and the risk of heart attack rises. Cells that line the artery walls are also affected; the ability of blood vessels to dilate is impaired, which can lead to high blood pressure.

One pivotal phenomenon underlying these changes is inflammation, already known to promote heart disease. Researchers at the University of Virginia School of Medicine in Charlottesville, for example, found high blood glucose linked to chronic inflammation in the blood vessel walls of mice.

Prevention Works

The first step to protecting your heart? Control your glucose. In the groundbreaking Diabetes Control and Complications Trial to determine the effects of glucose on complications, type 1 patients underwent intensive glucose control. A later study found these participants were 57 percent less likely to die from cardiovascular disease over the next 16 years.

Although the findings haven't been confirmed in those with type 2, high blood glucose levels have been proven to increase the risk of heart disease in both kinds of diabetes. Says David Nathan, M.D., professor of medicine at Harvard Medical School and co-chair of the type 1 study, "The major difference is that people with type 2 are generally older, heavier and more likely to have hypertension and abnormal cholesterol levels, all of which puts them at much higher risk for heart disease." For them, prevention is even more urgent.

Keep your heart healthy by following the ABCs of diabetes prevention: Aim for A1C levels below 7 percent, blood pressure below 130/80 mm Hg and LDL cholesterol below 100 mg/dl.

"Everyone with diabetes should know their numbers and goals, and get checked regularly," says John Buse, M.D., lead author of the ADA and American Heart Association's new joint statement on preventing cardiovascular diseases in people with diabetes. Start with a healthy lifestyle:

  • Nutrition Eat less fatty red meat, cheese, butter, processed foods and fast food; instead, choose more beans, vegetables, fruits and whole grains, along with lean meats and poultry, seafood, low-fat dairy and nuts. If you're overweight, cut some 250 to 500 calories a day until you've lost about 7 percent of your weight. "To do this right, ask your doctor to refer you to a registered dietitian," Dr. Buse says.
  • Exercise Each week, be sure to do a minimum of 150 minutes of moderate-intensity aerobic activity--swimming, biking, walking briskly. Exercise at least every other day.
  • Even if your doctor prescribes medication, don't give up on diet and exercise: A healthy lifestyle not only has broad positive effects on blood glucose levels, blood pressure and cholesterol levels, says Dr. Buse, but also helps you fight depression, function at your best and maintain your energy.
