Avandia and Actos Pose Increased Bone Fracture Dangers to Women Suffering From Type 2 Diabetes

A recent issue of the Canadian Medical Association Journal (CMAJ) reported the results of a study indicating that type 2 diabetes women who consume Avandia (rosiglitazone) or Actos (pioglitazone) are "at an increased risk of non-vertebral fractures with a near doubling in the risk of hip fractures."


The study, which was conducted by researchers at the University of East Anglia in Norwich, evaluated the risks of bone density degradation among women who consumed the diabetic therapy treatment. Of the 13,715 participants involved in 10 radomized clinical trials, a large portion were found to have "a significantly increased risk of fractures," according to the CMAJ article.

Scientists were able to identify an increase in bone mineral density loss among women patients specifically in the lumbar spine, which is the largest movable section of the vertebrae that is located above the tailbone.

The study found no risk of bone density loss among men, but women who were already at risk for developing bone fractures were said to be affected with one out of every 21 women developing the condition, while those women not at a predisposed bone fracture risk were said to be affected with one out of every 55 women developing bone density loss.

How Thiazolidinediones Work

Approximately 4 million Americans currently consumed a type of thiazolidineodiones. Thiazolidinediones, also known as glitazones, are type 2 diabetes treatments that include rosiglitazones (Avandia, Advandamet) and pioglitazone (Actos, Competact). The therapies "work by reducing insulin resistance and helping the body use the insulin it produces," according to information from the Medicines and Helathcare Products Regulatory (MHRA) organization, which is a regulatory body in the United Kingdom similar to that of the U.S. Food and Drug Administration (FDA).

However, the MHRA also reported safety concerns and side effects that have been associated with the consumption of these medications including the following:

* hypersensitivity
* cardiac failure
* hepatic impairment
* macular oedema
* bone fracture

Avandia Side Effects

Of the thiazolidinediones, is the commonly prescribed Avandia therapy from GlaxoSmithKline, which was released in May 1999. in addition to bone fracture as an Avandia side effect, there are several other risk factors associated with consuming the drug including:

* cough
* headache

* swelling, fluid retention
* cardiac arrest

Although Avandia has repeated made numerous headlines for the controversy surrounding its use, it remains on the market. Researchers who conducted the study explained that "these drugs need stronger warnings about potential side effects," according to a Washington Post article on the study.

If you currently have a canker sore or a few in your mouth you may be wondering what causes canker sores in the mouth. Perhaps you have had these annoying red, inflamed sores many times before. Canker sores are also called aphthous ulcers in the medical field, but I'll keep this simple.

A canker sore is a painful open sore in the mouth. It can be yellow or white and is usually surrounded by an inflamed reddened area. A canker sore is never a malignant (cancerous) sore. It is benign, meaning it is not cancer. It is also not a herpes sore (herpes simplex).

I've found that people who have gluten intolerance and/or have celiac disease have canker sores until they stop eating gluten. This could be one genetic connection, since celiac disease is genetic and passed on in the genes although the genes still need to be triggered by something unknown. One can carry the gene but not have full-blown celiac disease yet or ever.

Some experts think it's associated with the body's immune system. Here again it could also relate back to celiac disease because gluten flattens the villi in the small intestine and nutrients can't be absorbed so the immune system suffers and its defenses are broken down. So nutritional deficiencies could play a part.

Even if one doesn't have celiac disease or is not gluten intolerant, they may be eating a bad diet. The standard American diet is lacking in a lot of good phytonutrients, vitamins and minerals and we don't know what we're missing that could cause canker sores. It's thought that deficiencies in iron, folic acid, or Vitamin B-12 could play a part in developing canker sores.

The canker sores often crop up after a mouth injury due to dental work, getting braces doing aggressive tooth cleaning, or biting of the tongue or cheek.

Canker sores can also appear during emotional stress, during menstrual periods and when there are hormonal changes. Food allergies could play a part too. Then there is exposure in the mouth to various chemicals, irritating the sensitive tissues.

Scientists Have Discovered Two Cancer Drugs Capable of Reversing, Preventing Diabetes

Researchers at the University of California, San Francisco (UCSF) recently reported the results of a study that successfully reversed and prevented type 1 diabetes with the administration of two cancer treatments. The drugs, known as imatinib (Gleevec) and sunitinib (Sutent), are normally used to treat a variety of cancers including gastrointestinal stomach tumors (GIST).

During the study, researchers found that 80 percent of the time type 1 diabetes entered into remission among test mice and "worked permanently in 80 percent of those that go into remission," according to news reports on the study. Additionally, researchers found that by administering the two cancer treatments "before the onset of autoimmune diabetes, prevented development" of the condition.

The study, funded by the National Institutes of Health (NIH) and the Juvenile Diabetes Research Foundation, is considered a breakthrough for type 1 diabetes patients who have very few effective drugs available to them, according to experts at the UCSF Diabetes Center. Science Daily reported that research is expected to continue on the cancer treatments and the associated findings until a safe and efficient clinical trial for humans can be adequately developed.

Diabetes Types 1 and 2

Both type 1 and type 2 diabetes are unique and have distinct characteristics in their makeup. Additionally, medications for either condition are markedly different and cannot be taken for the opposite condition as severe health risks may occur.

According to a Science Daily news article, "type 1 diabetes is caused by an autoimmune response that destroys insulin-secreting cells in the pancreas," whereas the Mayo Clinic described type 2 diabetes as "a chronic condition that affects the way the body metabolizes sugar (glucose)."

Both conditions are, however, preceded by pre-diabetes, which is when "blood glucose levels are higher than normal but not high enough for a diagnosis of diabetes," according to the National Diabetes Information Clearinghouse (NDIC), which is a service of the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK).

Individuals with pre-diabetes are able to reduce their risk of either condition by participating in physical activity as well as weight loss. However, it is still important for individuals at risk for pre-diabetes and diabetes to not only speak with their physician on the disease, but to also know the signs and symptoms of diabetes, which include the following as described by the NDIC:

* blurry eyesight
* dry, itchy skin
* slow-healing sores
* unexplained weight loss
* urinating often
* excessively thirsty and hungry

Sweet Treatment For Diabetes From Ayurveda

According to Ayurveda, a holistic alternative medical science, the main cause of diabetes are fat, urine, and Kapha buildups due to foods, liquids, lifestyle, stress and others.

It generally believes that any disease caused is due to doshic imbalance. The doshas are the three humors that govern the human body, they are vata, pitta and kapha. It includes diabetes in the prameha category, which are a list of urinary disorders especially characterized by profuse urination with several abnormal qualities.

Classical Ayurvedic therapy for madhumeha (diabetes) begins with an assessment of the dosha imbalance. In all types of pramehas , kapha is vitiated, but in madhumeha, vata is often aggravated as well. Therefore, therapies will be directed at both vata and kapha simultaneously to restore the balance of the doshas.

By determining the causal imbalances at the basis of a disorder, an Ayurveda evaluation strikes the root causes of diabetes which are Overeating and consequent obesity, Excessive intake of sugar and refined carbohydrates, Overloading of proteins and fats which get converted into sugar if taken in excess, Excessive tension, worry, anxiety & grief, Hereditary factors.

Common ayurveda remedies include :

a) Diet restrictions -Avoid foods that increase kapha dosha like potatoes, rice, sugar, fatty foods and avoiding alcohol.

b) Exercise - Regular physical exercise is a must for diabetics. Practising yogic asanas like 'Halasan', 'Paschimottanasan" are found effective.

c) Panchakarma Therapy - The greatest benefit of Panchakarma is that it reduces the insulin resistance in the tissues in diabetics which is the primary goal in treating the diabetes.

i.e., Ayurveda also addresses the diet modification by eliminating sugar and simple carbohydrates, and emphasizing complex carbohydrates. Protein is limited, as excessive intake can damage the kidneys. Fat is also limited because there is often a deficiency of pancreatic enzymes, making fat digestion difficult. Since many diabetics have autoantibodies, Panchakarma is typically used for this cleansing the same. This begins with herbal massages and an herbal steam sauna, followed by fasting to cleanse the body. This is followed by an herbal purge for the liver, pancreas, and spleen. Colon therapy is next, first to cleanse the digestive tract and then to reconstitute the system.
Ayurvedic management of Diabetes depends on the strength of patient and type of diabetes he/she has. But some generalized management are mentioned below.

If the patient has sufficient strength panchakarma treatment can be started with snehana followed by vamana and virechana. Virechana can be repeated frequently. Dhanwantharam Gritham is apt for snehanam. Pizhichil is good for a week. Takradhara can also yield good results. Internal medicines that can be used are katakakhadiradi kashayam, aragvadhadi kashayam, nisadi kashayam, mehari dravakam and Nisha katakadi qwath. Apart from these ayaskriti and lodhrasavam are good.

Mehasamhari gulika, brihantmehantaka gulika, niruryadi gulika, svetagaunjadi gulika, haldi (turmeric), amalaki (gooseberry) choorna, nyagrodadhi churna,Brihat triphala churna, Madhumegha karpagm, nisosiradi tailam,or eladi tailam can be added to the kashayams as the case may be. Shilajit, vasntakusumakara rasa, nag bhasma, vanga bhasma, and chandraprabha vati can be used favorably.

Hyponidd tablets, Diabecon tablets, Mehanil tablets, Glucova, Nishakatakadi qwath tablet can be used on regular basis.

Sugar in any form like rice, especially from fresh grains, potato, banana, and other such cereals and fruits are to be avoided. Fat intake should also be avoided. Tamarid and salt intake should be minimal. All bitter things are of help in general. Vegetables like bitter gourd, drumstick, patola(sponge gourd), okra (ladies finger) are good. So is neem (Azadirachta indica) and bilva (Aegle marmelos). It is also advised to decrease all food intakes by three quarters of normal. Wheat, barely and all millets are good. Some cases recover fully just with exclusively meat diet. Don't use polished grains as bryan is good for diabetes.

Shadangapaniyam or water boiled with ekanayakam (Salacia reticulate) is excellent to quench thirst in diabetics. Exercise within limits is advisable, yogic exercise, particularly matsyendra asanam is useful.

Sleep during daytime, sex, not resting after food intake and excessive labour is to be avoided.

Painful Corns on Your Toes?

"My toes are hurting every time I put on a pair of shoes! Even my Crocs hurt! I've noticed some little red spots on my toes that seem to be growing. What's going on? I've tried to scrub them off but they hurt! I even tried some pads but they just made my shoes hurt more! Help!"

Sounds like she may have corns! Corns: such a simple name for a relatively complex problem. Corns can be extremely painful and actually are caused by several different foot ailments, so finding the root cause can take some detective work.

What is a corn?

The name "corn" came from the appearance of a bump that looks like a corn kernel on or between your toes. Corns are actually your body's defense against the rubbing of your shoes. They are layers of compacted, dead skin cells that your body makes to protect itself. Corns can be "soft" or "hard". Soft corns are usually seen in between the toes and hard corns are on the tops or tips of your toes.

What causes a corn?

These annoying bumps are caused by repeated friction and pressure from skin rubbing against a bone or against an irregularity in a shoe like a seam. Either your toes are crooked or the shoe just does not fit properly. The friction can cause significant pain from pressure points which makes wearing shoes very uncomfortable.

Why do my corns hurt even without shoes?

If you have pain in a corn, even when you do not have shoes on, you have most likely developed bursitis which is inflammation of the joint under the corn. Bursitis is often treated with an injection of anti-inflammatory into the inflamed area. This type of pain should be evaluated by your podiatrist, because the other reason for pain without shoes is an infection in or around the corn. Toe infections can become serious and may even lead to amputation in some patients. Don't ignore your toe pain!

How are corns treated?

The pain from corns often may be relieved by felt, foam or silicone padding on the affected areas. A good moisturizing cream can also help. Your podiatrist can scrape or "debride" the corn to reduce the thickness, but remember never to cut corns with any instrument at home, and never apply home remedies or "corn remover patches". These invite infection and can make your corns a foot emergency!

There are only two permanent treatments for corns:

1. Surgically correct crooked toes also known as hammertoes. Or 2. Find the culprit shoe and stop wearing it!

Physio Treatment in Ankylosing Spondylitis by Jonathan Blood Smyth

The inflammatory arthritis diseases or spondyloarthropathies include various diseases such as Ankylosing spondylitis, the arthritis of bowel disease, reactive arthritis and the arthritis associated with psoriasis. The typical linking features of these diseases are enthesitis (an inflammation at the bone/ligament junction) and the presence of HLA B27, a gene on white blood cells. The inflammation at the entheses can develop into fibrosis and eventually to fusion of the joints from bone formation.

AS is the commonest of the spondyloarthropathies and its occurrence varies with the occurrence of the HLA B27 gene in the population, AS being less common in the tropics and more common in northern European countries. 0.1 to 1.0% of people are affected but this varies with latitude and is more common in white people. About 1-2% of people with the HLA B27 gene actually develop AS but this becomes 15-20% likelihood if they have a first degree relative with the disease.

AS occurs more often in males, with a 3:1 ratio, but females may suffer much milder disease or have minor symptoms which are not diagnosed. AS is a disease of young men, with most presenting before they are 40 years old, and 10-20% presenting with the disease before they are 16. On average people get the symptoms of the disease around 25 years old and are rarely diagnosed when over 50 years of age. The condition is often missed initially as it presents as low back pain so it is important to take a proper history. A useful question is "how does your back feel when you wake up" and AS patients will all say "very stiff".

Low back pain is the major diagnostic alternative but AS patients are generally younger and the inflammatory process leads to different symptoms:

Morning back stiffness lasting half an hour and often longer Back pain improved with exercise Back pain worsened with rest Night pain later on in the night Other joints may be affected Fatigue is common Active inflammatory disease can cause systemic affects such as unwellness, weight loss or fever

Physiotherapy examination of the spine in an AS patient usually uncovers significantly reduced ranges of spinal movement from normal, with perhaps a reduced lumbar lordosis and an increased thoracic curve. Neck movements may also be limited in later stages and a reduction in chest expansion noted due to rib joint involvement. Peripheral symptoms occur in around a third of patients and the physio will palpate the tender areas, searching for evidence of enthesitis in the insertions of the Achilles tendon and plantar ligament of the foot. These are areas of high mechanical stress and commonly affected.

Physiotherapy assessment of patients with Ankylosing spondylitis begins with postural assessment of spinal deformity and perhaps flexed knees. Increased cervico-thoracic kyphosis is common with a poking chin and rounded shoulders. General spinal mobility is obviously limited in normal movement and assessments are made of the lumbar, thoracic and neck ranges of motion. Other standardized measurements are taken to gage the progress of the disease or the affect of therapy. How Does A Disc Herniate? Dr. Anthony Abbruzzese states that "A healthy disc is tough, resilient and resembles gristle. As a disc degenerates it looses its toughness and assumes the consistency of soft crab meat" This is the process of degeneration. As degeneration progresses the structural integrity of not just the disc but also that of the vertebral segment is compromised. This situation leads to segmental "dysfunction" much like "shimmy" in a car body. This "shimmy" directs stress to the zygoapophyseal joints (facet joints) causing them to also degenerate.

Disc degeneration is easily recognized in post mortem tissue but the magnetic resonance imaging (MRI) technique is a particularly sensitive means of showing this. The reason for this is that "normal" discs are about 85% water. As discs degenerate they loose their water content. An MRI scan is actually a scan of hydrogen ions in the body. Since almost all hydrogen ions are found in body water (H2O) a MRI is basically a very sensitive "water content" scan of all body tissues.

Intervertebral discs are one of only two body structures in adult life which do not have their own blood supply. The other one is the cornea of the eye. The cornea obtains its nutrition from tears while the disc obtains its nutrition from the diffusion of nutrient solutes across the porous endplates of the adjacent vertebrae - aka -the more we move around and stay active, the more hydtarted our discs are.

The diffusion of nutrients across the endplate of the vertebrae can be enhanced by "pumping" activities (types of exercise) and by decreasing intradiscal tension (by segmental traction or distraction). In the case of individuals born with abnormal endplates (i.e. juvenile discogenic disease) the challenge to maintain healthy discs is greater and the liabilities of poisoning discal cells (i.e. smoking) is greater. Disc Degeneration versus Degenerated Tires: In many ways the simile between degenerated discs and degenerated tires is valid. In order for a human being to be successful for long-term (about 80 years) ambulation on a high gravity planet the individual has to have been careful in parenteral selection so that their spinal column genetic makeup will be adequate to deal with the insults and injuries along the way. If initial fabrication of the spine is flawed incapacitation and disability will result. Tires, if sound, can be recapped. Spines only continue to be worn down by the daily insults of life and occupation, occasional traumas, and the process of aging. Genomic testing to determine spine liabilities early in life will be the future equivalent of tire testing before use. Its a shame we don't have this now because because valid self-administered spine health maintenance programs presently exist. Tires are stressed when under inflated and run at high speeds in high temperature environments. In the same way human spines are stressed by labor intensive jobs as well as occupations such as trucking where compressional loading and vibratory factors conspire to injure discs. The simile with tires ends with the observation that humans can actually, on a daily basis, perform health maintenance programs capable of enhancing and improving spine nutrition and function whereas tires can only become more run down over the passage of time. How Does Disc Herniation Start? Unless due to acute trauma a typical disc herniation begins with an unhappy disc. The circumstances that promote this state of affairs is, as can be seen, are usually multiple leading to a loss of disc integrity. Once degeneration exists it usually doesn't take much for a disc herniation to start. Very often the first step in this process is a twist "injury" producing a tear in the disc annulus. Why Is There Back Pain? In the September 2000 Issue of the Oprah Magazine an article "The Real Source of Your Back Pain" written by Emma F. Segal quotes physician Art Brownstein. His answer to the question "What causes most back problems?" was "Muscle strains and sprains." Another quote from Doctor was: "It is a common misconception that back pain starts with a disc problem." These statements are poor responses to the questions asked and are misleading to the public.

It is an anatomic fact the the surface of the brain given over to representing the back is remarkably small. This poor representation is the reason that back pain is non-specific and so poorly localized in comparison to large brain areas for fingers, tongue, etc., allowing more specific localization.

There is no question but that one can "strain or sprain" the ligamentous or muscular components of the spine but most back pain, spasm, and incapacitation are secondary to degenerative changes in the spinal column reflecting congenital abnormalities and well as acquired insults and injuries.

There are many treatment options for a herniated disc ranging from chiropractic to physical therapy to pain medications to spinal decompression to surgery to manipulation under anesthesia to epidurals...and the list goes on and on and on. So which treatment option should you choose? I would start by consulting with your family doctor, chiropractor, or other health care provider.
