We have all heard this before. As teenagers from our school coaches who would constantly belt out the proverbial “no pain, no gain” concept, this ridiculous idea that unless we exercised like crazy people, we would not likely see the benefits of our effort, and then from family and friends commenting on our lack of success in weight loss. “If there is a will, there is a way” we were told, or in other words, the reason we were overweight was simply because we just didn’t really want to lose weight.

That is not to say of course that to achieve any of the above goals doesn’t require effort and dedication on our parts. It does.. And lots of it. But research has shown that the majority of people with substantial weight problem have become pre-conditioned to the inevitability of their current situation and thus, their attempts to lose fat become more compromised with the failure of each diet. Putting them (or yourself) down for being fat or overweight may feel like a powerful motivational tool in the reverse psychological perverse sort of way, but in fact, it actually hampers any efforts to change our eating habits.

Talk to any weight loss doctors who were once overweight and successfully lost their excess weight and fat, and chances are you’ll be told that food cravings and weight gain are not always the result of a lack of willpower.

! Most of us who are chronic dieters have one thing in common then. We are not short of willpower (feels good just writing this again!) and likely suffer from a metabolism that produced too much insulin due to an addiction to carbohydrate.

So what’s the deal with insulin? Well:

Insulin tells your body when to eat
Insulin sends food energy wherever your body needs it
Insulin instructs your body to save food energy which is then stored in fat cells for the time when food becomes unavailable.

This overproduction of insulin levels caused by the over absorption of carbohydrates causes a chain reaction which then leads to hunger pangs. Put in another way, the more carbohydrate you eat, the more insulin your body produces. This causes an imbalance which then leads to a cycle of overpowering cravings for more carbohydrates, such as breads, pastas, snack foods and cakes which then leads to even more insulin and so on..

You are a carbohydrate addict if:

You are hungrier after breakfast and before lunch time then if you had skipped breakfast all together.

You are tired and sluggish after a large meal (particularly in the afternoon)

You have been on diet after diet and have as much weight to lose (or even more) as when you started.

You want to eat when you are bored or tired

You feel that you are not satisfied, even after a meal

You find it harder to lose weight then when you were younger.

If you answered “yes” to any two of the above six question then you are mildly carbohydrate addicted. You are moderately addicted if you have answer “yes to three to four questions, and you have a severe carbohydrate addiction if you said “yes” to five or all of the above questions.

Top 7 Tips to Treat and Prevent Bone Spurs

4. Apply Heat

Keep inflammation in check with daily heat applications. Hold a heating pad or a hot-water bottle, as warm as can be tolerated and wrapped in a towel, on the affected area for 10 to 15 minutes four to five times a day.

5. Reduce Inflammation

Acute pain can be reduced by applying ice to the inflamed area four to five times a day. Hold an ice pack wrapped in a towel or cloth on the area for 10 minutes, then remove the pack for another 10 minutes. Repeat this procedure several times, or until the throbbing subsides.

6. Try Padded Cell

When you are buying new shoes, select supportive, well-padded shoes – the kind with shock-absorbing insoles and a rigid heel support. In addition, shoes with laces will provide more support than slip-on shoes or sandals.

7. Avoid Flat Shoes

Unless you find that they really provide adequate support and shock absorption, steer clear of flat shoes. They stretch the ligament on the bottom of the foot even farther. Sandals, sling-backs, and canvas tennis shoes are a bad choice for people with heel spurs. These styles provide little to no heel support and control.

How To Stop Nose Bleeds, Is Cauterization A Permanent Solution To Nose Bleeds?

During the Nose Bleed

Thumbs Up Method

1. Stay calm, slowly taking deep breaths.
2. With your hand (same hand as the bleeding nostril,) make a 'thumbs up' sign.
3. Press your thumb against your nostril's side, not so hard that it hurts, but firmly
enough to flatten the nostril.
4. Stay still, leaning forward slightly.
5. Stay in place for 5-10 minutes, breathing normally.
6. Slowly release the pressure; it may feel a bit weird as the blood flows back into your nasal vessels.

Pinching Method

1. Pinch your nose just below the bridge. There is a vein just below the nose bone that is the culprit in 99% of bloody noses. Pinching puts pressure on it, which arrests the bleeding and speeds the clotting process.
2. Find a bathroom as you continue pinching. Now that you have slowed the bleeding by pinching, you should find a bathroom where you can clean up once the bleeding has stopped.
3. Keep applying pressure for at least 5 minutes at a time. Don't check to see if it is still bleeding over this period of time as it is important to keep continuous pressure. After this period of time let go briefly to see if the bleeding has stopped. If not, give it another 5 minutes. (This is also a good time to quickly wash any blood off your hands and get a paper towel or toilet paper to pinch with so that blood gets on the paper and not your hand.) If it is, continue pinching. Don't check every 30 seconds, as the key is constant pressure.

Pressure Method

1. Find the two very slight depressions on the back of the skull, approximately four finger-widths from the base of the skull (in line with the tops of the ears) and four finger-widths from the mid-line of the back of the skull. If you had eyes in the back of your head, this is where they would be.
2. Press the spots firmly, but gently, and if you have connected correctly, the bleeding should stop immediately. Keep up the pressure for about five minutes and then release. If the bleeding starts again, just repeat the process, but hold it longer: you may have to keep up the pressure for ten to fifteen minutes to stop it completely.

Upper Lip Method

1. Roll up a piece of gauze or tissue into a "cigar shape" approximately 2 inches long and a little thicker than a pencil. Folding it into a small, thick square also works well.
2. Wedge the tissue under your upper lip where it's tight and close your lip over it.
3. Apply light pressure by compressing your lip over the wad. Tilt your head forward.

Remedies

1. The green leafy vegetables are a source of Vitamin K, which is needed in trace amounts to help blood to clot. This Vitamin is needed in such small amounts that it's difficult to find it made in tablets all by itself. You need to eat green leafy vegetables regularly.
2. Taking ONE 1000 milligram Vitamin C tablet and ONE cayenne pepper capsule each day with your largest meal, and stay at that dose until you feel comfortable and have regular, normal bowel movements. Remember to drink lots of water at least 20 minutes before your meal, and also after about an hour after your meal. It's best not to drink a lot of water with your meal, as this will dilute your digestive juices and make digestion more difficult--unless, of course, you are severely dehydrated.

Summary

It is not common practice for doctors to check patients with a history of epistaxis for genetic disorders, vitamin K deficiencies and/or blood vessel constriction etc. However, these would all be highly logical possible causes to consider and evaluate in patients. Otherwise it will be an undiagnosed nose bleeds. Nuke Your Microwave Oven!

Toss the microwave ovens - the sooner the better! I would imagine that most of us have seen warnings about using microwave ovens for some time. The first ones I recall were way back in the radical seventies, when I learned that our standards for allowable microwaves were 100's of times less stringent than the world leading authorities in microwaves - the Soviet Union (the same folks that constantly bombarded our embassy in Moscow with microwaves). I also read reports that the incidence of cataracts among microwave users was alarmingly high - especially those that stood close to the microwave while it was in operation. And what made the cataracts really stand out was that they were "anterior cataracts" which formed further within the eye than normal cataracts which formed near the lens surface. Hard to miss the connection with microwave ovens, which cook food from the inside out.

I recall seeing a Dr. Edward Fujimoto (manager of the Wellness Program) at the Castle hospital on a TV program where he explained this health hazard. He was talking about dioxins and how bad they are for us. He said that we should not be heating our food in the microwave using plastic containers.

This applies to foods that contain fat. He said that the combination of fat, high heat and plastics re leases dioxins into the food and ultimately into the cells of the body. Dioxins are carcinogens and highly toxic to the cells of our bodies. Instead, he recommends using glass, Corning Ware, or ceramic containers for heating food. You get the same results, without the dioxins.

So such things as TV dinners, instant ramen and soups, etc., should be removed from the container and heated in something else. Paper isn't bad but you don't know what is in the paper. It's just safer to use tempered glass, Corning Ware, etc. He said we might remember when some of the fast food restaurants moved away from the foam containers to paper. The dioxin problem is one of the reasons.

To add to this, Saran wrap placed over foods as they are nuked, with the high heat, actually drips poisonous toxins into the food, use paper towels.

I had heard those warnings before, at least the ones about dioxins from the plastic. For proof that microwaves do something to plastic, just cook a dinner a bit longer than recommended and see where the plastic melts and turns to some kind of slag, and also mixes in with your food! Of look at some of your old Tupperware so-called microwavable dishes that have been used a long time and look at the mottled and dimpled surfaces where repeated zapping has altered the plastic. What I had not seen before though was the warning about freezing water bottles.

After getting this latest warning, I decided to not just put my blinders on and continue making carcinogens. I thought, OK, I will just use corning ward or tempered glass. Just to make sure, I did some checking on microwaves. What I found was startling!

To put it quite simply, whereas traditional cooking heats food with temperature and cooks food from the outside in, micro waved food is cooked from the inside out by bombarding the food with microwaves. Furthermore, microwaving alters food and creates chemical compounds that the human body has not evolved or adapted to handle.

I found that scientific clinical studies had been conducted by the Russians (THE microwave authorities) the Swiss and the Germans. In some instances these studies have been suppressed by industry, particularly where industry had charges brought against a researcher in Switzerland, and, after initially having the researcher convicted, fined and given a gag order, industry lost on appeal and had to pay the researcher for damages.

Here are the consensus conclusions of the studies conducted in the three countries:

1). Continually eating food processed from a microwave oven causes long term - permanent - brain damage by "shorting out" electrical impulses in the brain [de-polarizing or de-magnetizing the brain tissue].

2). The human body cannot metabolize [break down] the unknown by-products created in micro waved food.

3). Male and female hormone production is shut down and/or altered by continually eating micro waved foods.

4). The effects of micro waved food by-products are residual [long term, permanent] within the human body.

5). Minerals, vitamins, and nutrients of all micro waved food is reduced or altered so that the human body gets little or no benefit, or the human body absorbs altered compounds that cannot be broken down.

6). The minerals in vegetables are altered into cancerous free radicals when cooked in microwave ovens.

7). Micro waved foods cause stomach and intestinal cancerous growths [tumors]. This may explain the rapidly increased rate of colon cancer in America.

8). The prolonged eating of micro waved foods causes cancerous cells to increase in human blood.

9). Continual ingestion of micro waved food causes immune system deficiencies through lymph gland and blood serum alterations.

10). Eating micro waved food causes loss of memory, concentration, emotional instability, and a decrease of intelligence."

Perhaps instead of thinking of the minutes we save from the microwaves, we should think of the years and the nutrition we may be losing! My suggestion: if you want to save time, try a good convection oven. Whatever you cook with, do not overcook. It costs nutrients and causes carcinogens, even in foods besides meats

What Is Hyaluronic Acid And How Does It Work For Arthritis?

Hyaluronic acid (HA) – also known as hyaluronan- is found in all tissues and body fluids. What this article will describe is what HA does in the joints, particularly the knee.

HA is a major component of synovial fluid. Synovial fluid is the naturally-occurring lubricant found in joints.

HA has a variety of functions within the joint including lubricating effects, water balance, and stress distribution through what are termed “viscoelastic properties.” What this means is that at low levels of impact (standing and walking), HA has mostly lubricating properties. At high levels of impact (running and jumping), HA functions to cushion the joint.

Hyaluronic acid (HA) is often used as a treatment for osteoarthritis (OA), particularly in the knee. HA therapy supplements HA in the knee, restoring the synovial fluid's ability to cushion, lubricate, and protect the knee joint.

A number of different mechanisms of action are thought to be responsible for the beneficial effects of HA. These include fluid replacement, inducement of the body’s own HA production by the knee joint, increase in cartilage synthesis, reduction in cartilage degradation, inhibition of inflammation, protection of the cells that make cartilage, and decrease in pain through inhibition of pain stimulus on nerve fibers within the joint.

Osteoarthritis is the most common form of arthritis. Its hallmark is the progressive breakdown of cartilage. As cartilage becomes progressively more damaged, changes in the HA within the joint also occur. Both the concentration as well as the molecular weight of HA undergo change.

The reduced concentration and molecular weight of HA lead to worsening of cartilage damage, increase in inflammation within the joint, and further reduction of function of synovial fluid.

Synovial fluid in patients with OA no longer performs its normal viscoelastic function. This perpetuates a cycle of reduced cushioning and lubrication leading to more damage and inflammation, and so on.

Both the American College of Rheumatology and the American Academy of Orthopedic Surgeons list HA therapy as an alternative and supplement to oral drugs for patients in whom non-drug therapy and treatment with acetaminophen (Tylenol) has failed, especially if non-steroidal anti-inflammatory drugs (NSAIDS) and COX-2 inhibitors are contraindicated.

The advantage of HA is not limited to just restoration of the synovial fluid's ability to cushion, lubricate, and protect the knee joint. HA also leads to pain relief and improvement in knee function and health-related quality of life. Finally, it is a localized therapy with no systemic side-effects.

There is also some data that HA therapy may actually postpone the need for knee replacement surgery in some cases.

Side effects of HA are minimal. The most common are local injection site reactions. Different forms of HA are available and these include Hyalgan, Synvisc, Supartz, Orthovisc, and Euflexxa. Most HA products are derived from chicken sources.

The exception is Euflexxa which is bioengineered from a different source.

Different injection schedules are required for the different preparations. Some require weekly injections for 3 weeks; others are weekly injections for 5 weeks.

The important things to remember are that HA should be administered by an experienced rheumatologist or orthopedic surgeon. Either ultrasound or fluroscopy should be used to ensure proper location of the needle and accuracy of HA injection.

Contraindications include active infection of the joint or skin near the joint, history of sensitivity to chicken prodcuts (except for Euflexxa), and prior allergic reaction to HA.

What is the cause of the disease?

The herpes simplex virus, Streptococcus pyogenes (GABHS) and Epstein-Barr virus (EBV), cytomegalovirus, adenovirus, and the measles virus cause most cases of acute pharyngitis and acute tonsillitis. Bacteria cause 15-30 percent of pharyngotonsillitis cases; GABHS is the cause for most bacterial tonsillitis. Whether due to viruses or bacteria, the infection is spread from person to person by airborne droplets, hand contact or kissing.

There are many different individual viruses and bacteria that can potentially cause tonsillitis. For example the Epstein-Barr virus, which is the cause of glandular fever (infectious mononucleosis), is a common cause. It is typically seen in young people, particularly in situations such as university halls of residence when person-to-person spread is made easier by numerous close contacts.

How is Tonsillitis diagnosed?

Tonsillitis can be diagnosed by performing a rapid strep test, also called a throat culture. To perform the throat culture, the doctor will use a long cotton swab to swipe off some of the stuff on the surface of the back of your throat. The doctor will then test the "stuff" on the cotton swab. This test will determine whether you have tonsillitis and whether it is caused by a bacteria or a virus.

Treatment

If your tonsillitis is viral, you can take medication to relieve the symptoms, but only time will make the virus go away.

If your tonisillitis is bacterial, your doctor may treat you with antibiotics.

Preventing Tonsillitis

To prevent tonsillitis, avoid letting your child near anyone who already has tonsillitis or a sore throat. Make sure to practice good hand-washing habits, and teach your child to do the same. If you are concerned that your child has the symptoms of tonsillitis, or has been exposed to someone who has been infected, talk with your child's doctor.

Stretching don’t take much time, but they ease neck stiffness. And you can do them at home or at work. One example: Turn your head all the way to one side, then back to the center so you are facing straight ahead, and then all the way to the other side. Look down at the floor, bring your head back to a straight-ahead position, next look up at the ceiling, then bring your head back. Repeat a few times a day, as needed.

3. Take A Load Off Your Shoulder

A heavy purse on one shoulder strains your neck as well as your back and shoulders. Switch to a fanny pack or backpack or carry your pulse across your chest. Load heavier belongings into a suitcase on wheels, as do many flight attendants and travelers. Or buy the wheels and a couple of bungee cords from a discount store and roll your luggage along.

4. Tilt Your Chair Back

Often, neck pain is caused by jutting your head forward. The solution? Bring your reading material to you. When you read or watch television, recline your chair so that your head is supported by the back of your chair or a wall. It is fine to sit on a couch or an easy chair, as long as you change positions frequently, so that your neck doesn’t get stiff.

5. Adjust Your PC Monitor

If you work at a computer, make sure you don’t have to crane your neck to look at the monitor. Adjust your monitor so it is at eye level.

6. Get A Copy Holder

You can easily strain your neck from looking back and forth from the computer to copy off to the side and lower than the screen. A copy holder attached to the monitor at eye level will allow you to read documents without straining your neck.

7. Trade Up To A Down Pillow

To keep from waking up with a stiff neck, choose a pillow made of down or other pliant material or one of the orthopedic pillows instead of solid foam rubber. A good pillow follows the contours of your neck instead of leaving your neck unsupported. And sleep on your side or back, not your stomach.

In fact, the researchers went so far as to say that interval training results in the "spot reduction" that all men and women try to achieve with abdominal training.

When interviewed, Professor Steve Boucher, one of the Australian researchers, explained, "high intensity intermittent exercise may result in greater fat loss in the abdomen". That means that unlike doing hundreds of crunches, interval training can work to burn belly fat over everything else.

This is incredible news for all men and women who want to achieve a lean midsection or even 6-pack abs. While most men and women spend all their time on abdominal exercises, the latest research clearly shows we should be doing interval training, not slow cardio or endless crunches. Intervals are the best way to lose stomach fat.

Fitness experts in North America have been one step ahead of this research. In fact, most fat loss programs these days include an interval training program. If your workout doesn't use interval training, you could be missing out on maximum belly fat burning. Fitness experts in North America have identified cardio as having what they call a "dark side". This refers to the increased risk of injury and lack of results from slow cardio.

The short, burst exercise workouts of interval training use more intense effort to get more results in less time than cardio. And more importantly, the workouts are over in less time. The researchers theorized interval training works better because it boosts fat burning hormones, while cardio does not. The Australian research group believes that men can also burn fat with interval training.

If you are new to interval training, you should start conservatively. Begin by doing 1 minute rounds of interval training at a slightly harder than normal exercise pace. Then spend 1 minute at a slow pace as a recovery interval. Do up to 6 intervals per workout, going between the hard exercise and easy exercise portions for a total of 12 minutes. Always include a 4-minute warmup and 4-minute cooldown to complete your 20 minute interval training session.

As your fitness improves, you can increase your interval intensity. Perform this type of workout 3 times per week after your resistance training workout and you will lose stomach fat, burn off your belly, and sculpt your body to develop a lean mid-section and toned arms.

Can Rooster Comb Injections Prevent Knee Replacement?

Osteoarthritis (OA) of the knee is one of the most common problems seen by both rheumatologists as well as orthopedists in the office. The standard forms of therapy include maintenance of proper weight, exercise, application of cold, bracing, analgesics (pain killers), anti-inflammatory medicines, steroid injections, and viscosupplementation. This latter type of treatment involves the injection of a lubricant directly into the knee.

These lubricants consist of a purified form of a protein called hyaluronic acid (HA). These preparations are effective in relieving pain. pain relief from OA of the knee starts between the 5th and 13th week follwoing injection. While HA is felt to be effective for pain relief, it has been unclear as to whether there are any other benefits.

Two recent studies have demonstrated that viscosupplementation may actually forestall the need for eventual knee replacement surgery and also may represent a long term cost savings.

The first study from Louisiana State University surveyed patients from a large orthopedic practice. A total of 863 patients (1187 knees) were evaluated. All patients had grade 4 changes, meaning they had “bone on bone” and were candidates for total knee replacement.

Using survival analysis of the data, it was estimated that total knee replacement was delayed approximately 3.8 years in 75 percent of the knees receiving viscosupplementation (Waddell DD, et al. J. Managed Care Pharm. 2007; 2:113-121.)

In another study, investigators in Thailand studied one hundred and eighty three patients with knee OA (208 knees) who failed conservative treatments and did not have contraindications for surgery were enrolled. All patients were treated with one course of three hyaluoronic acid injections at weekly intervals and followed up for a minimum 2-year period. In case of successful treatment (response group), repeated doses were recommended. If the patients did not improve within one month after completion of the injections, they would be classified as a non-response group and total knee replacement surgery was considered. Cost of direct medical costs (drugs), hospitalization, and resource utilization were recorded and analyzed.

They concluded that IA-HA (joint injection with HA) should be considered as a medical intervention before surgical procedures in knee OA patients who failed conservative treatments. Even though the cost of IA-HA treatment would increase the total costs of treatment and some patients might fail, it was only 6.44% of the total costs. On the other hand, if patients responded to IA-HA treatment, then the surgical procedures were not required. This treatment would represent a savings of 63.26% of total costs. (Turajane T, et al. J Med Assoc Thailand. 2007; 90;1839).

While, the exact analyses and conclusions of these two studies are different, they do suggest that HA may be cost effectve in delaying the need for total knee replacement.

There are five HA preparations available. They are Hyalgan, Synvisc, Supartz, Orthovisc, and Euflexxa. Only Euflexxa is not derived from a chicken source.

The number of injections required varies from 3-5 depending on the preparation used. Hyalgan and Supartz generally are given as weekly injections for 5 weeks while Synvisc, Orthovisc, and Euflexxa are given as weekly injections for 3 weeks.

Thes injections should be administered using either fluoroscopic or ultrasound needle guidance to ensure accuracy.

Side-effects are minimal when administered by trained specialists.

HA injections are worth a try even with grade 4 knees if patients wish to delay surgery.

For decades, thousands of people looking for “the fountain of youth” flocked to the spa-like clinic of Dr. Ana Aslan in Bucharest, Romania. They came to the spa not only to rest and relax but to get their supply of Gerovital H3 (GH3), a drug popularized by Aslan in the 1950s.

Although Aslan left this world in 1988, her legacy remains. Gerovital is still the main attraction of several spas in Romania and the promises are plenty. Regular shots of the drug will supposedly reverse aging and prevent a wide range of diseases. For elderly tourists in search of a cure-all, Gerovital guarantees smooth skin, improved memory, increased vitality, freedom from depression and heart disease, and a pain-free life.

These claims have been swallowed by the rich and famous. Among Aslan’s many clients were John F. Kennedy, Marlene Dietrich, Kirk Douglas, and Salvador Dali. Sylvester Stallone also reportedly takes GH3 shots to remain sexy. Some doctors prescribe Gerovital for impotence. Others use it to make hair grow and treat mental disorders.

But it’s unlikely that Gerovital will deliver what it promises. The reason is simple: Gerovital’s active ingredient is procaine or novocaine, a numbing agent similar to cocaine. Procaine is an excellent anesthetic widely used by dentists but it has no anti-aging effect.

The reason some people swear by Gerovital is because oral procaine dilates the superficial blood vessels of the face and scalp, causing a “flush”. This strange feeling creates the impression that the drug works. That plus the program of exercise and much personal attention one gets from Gerovital spas can easily make one feel better. However, studies show that Gerovital is nothing more than an anesthetic.

“GH3 is a two percent procaine hydrochloride solution to which minute amounts of the following have been added as ‘stabilizers’ or ‘buffers’: Benzoic acid, potassium metabisulfite and disodium phosphate. The procaine (which is the same novocaine anesthetic that you get when you go to the dentist) is supposed to be the active ingredient. Procaine, however, is rapidly hydrolized (decomposed) once it enters the bloodstream, even after buffering or stabilizing agents are added, and there is no evidence - or even any good reason to suspect - that these additives can sustain the activity of procaine long enough to produce the claimed benefits,” according to Dr. Sheldon Saul Hendler in “The Doctors’ Vitamin and Mineral Encyclopedia.”

To this day, Gerovital remains in the US food and Drug Administration’s (FDA) list of unapproved drugs. The agency automatically confiscates commercial shipments of oral and injectable forms of Gerovital and its sister products – KH3, Zell H3, GH3 Cream, etc. Still, many products continue to enter the United States from various areas under the guise of “free medical samples”.

“Over the past two decades the US FDA has taken several actions against companies marketing Gerovital but the drug always resurfaces. Despite lavish health claims, its main ingredient has been shown only to have mild antidepressant effects. In some brands tested by the FDA, even that ingredient was missing,” said the editors of Consumer Reports.

What is this condition?

Neurogenic bladder refers to all types of bladder problems caused by disruption of normal nerve impulses to the bladder. The complications that follow neurogenic bladder include lack of bladder control, failure to completely empty, urinary infection, stone formation, and kidney failure. A neurogenic bladder can be spastic or flaccid.

What causes it?

At one time, experts thought neurogenic bladder was caused by spinal cord injury. Now it appears to be linked to such conditions as acute infectious diseases, dementia, heavy metal toxicity, certain types of cancer, and many additional disorders.

What are its symptoms?

Neurogenic bladder produces a wide range of effects, depending on the underlying cause and its effect on the structural integrity of the bladder. Usually, the person with this disorder has some degree of mcontmence.

If the person has a spinal cord lesion, a spastic neurogenic bladder may produce involuntary or frequent scanty urination without a feeling of bladder fullness and, possibly, spontaneous spasms of the arms and legs, hypertension, and headaches. Flaccid neurogenic bladder may be linked to overflow incontinence, diminished anal sphincter tone, and a greatly distended bladder that the doctor can feel. The person may feel no bladder fullness because the senses are impaired.

How is it treated?

The doctor will work to maintain the integrity of the upper urinary tract, control infection, and prevent urinary incontinence through evacuation of the bladder, drug therapy, surgery or, less commonly, neural blocks and electrical stimulation.

The person may be taught to empty his or her bladder by applying manual pressure over the lower abdomen. He or she may be taught how to insert and remove a catheter. Generally, a man can perform this procedure more easily, but a woman can learn self-catheterization with the help of a mirror. Intermittent self-catheterization, in conjunction with a bladder-retraining program, is especially useful for people with flaccid neurogenic bladder.

If conservative treatment fails, the doctor may use surgery to correct structural problems by modifYing the bladder neck, widening the urethra, removing part of the sphincter muscle, or building a new route for urine. Implantation of an artificial urinary sphincter may be necessary if permanent incontinence follows surgery for neurogenic bladder.

Pernicious anemia is a condition caused by insufficient red blood cells due to a lack of vitamin B12 in the body. In the anemic person, the blood cannot carry enough oxygen to the cells of the body. The blood cells are larger than, and do not split like healthy ones.

The name "pernicious anemia" was coined in 1872 by the German physician Anton Biermer because it was often fatal as there were yet no treatments available. The disease, even today, can be severe if it is not treated in time. It is especially common in older adults.

Vitamin B12 deficiency can be caused by a number of factors. A diet low in this vitamin, like a strict vegetarian diet that excludes meat, fish, dairy products and eggs is the main culprit. However, most of the average North American diets of today are sadly lacking in B12. Breastfed infants of strict vegetarian mothers can develop anemia in a short time because they dont have enough vitamin B12 stored in their bodies.

Chronic alcoholics and the aged are prime targets for this disease as they tend not to eat nutritious meals and have digestive troubles. The main causes are insufficient stomach acid, or Intrinsic Factor, to digest food in the intestine, parasite infections, an overabundance of intestinal bacteria, and medicine. B12 is very hard to digest and is dependent on sufficient Intrinsic Factor to be absorbed from the intestines into the blood stream. Celiac disease, a genetic disorder of intolerance to gluten, and Crohns disease, an inflammatory bowel disease, can also precipitate anemia.

The main risk factors for developing pernicious anemia include a family history of pernicious anemia, diabetes, a malfunctioning thyroid, and intestinal disorders.

The most common symptom of anemia is feeling tired and weak. As anemic blood is not carrying enough oxygen, it causes the heart to work harder to get enough oxygen to the bodys organs and tissues. There is a plethora of other symptoms. These include a smooth bright red tongue, pale or yellowish skin, low-grade fever, mental changes, nervous disorders, dizziness, shortness-of-breath, heart palpitations, tingling and numbness in hands and feet, giddiness, and digestive disorders.

As mild to moderate anemia may have no signs or symptoms, it may initially be hard to discover.

Anemia is usually diagnosed by a general practitioner by means of checks on your family and medical history, physical examinations, and diagnostic tests and procedures including a complete blood count (CBC).Depending on the symptoms and/or conditions, neurologists, cardiologists, hematologists and gastroenterologists may diagnose your condition.

Typical treatment for pernicious anemia is lifelong vitamin B12 injections. A lack of folic acid, however, can also cause anemia. Due to the lack of vitamin B12 and folic acid, the arteries of an anemic person are clogged with homocysteine as well. Without treatment, therefore, pernicious anemia can cause serious problems and can be fatal.

People with pernicious anemia also have an increased risk for developing stomach cancer and should be closely monitored by their health professionals.

Wine in moderation has been known to lower the risk of heart disease. It doesn't seem to be wine specifically, but the stress relieving properties of alcohol, as long as they are consumed in moderation. Gradual decrease of testosterone hormones in men as they reach a certain age is referred to as andropause, or male menopause as it is called. Thus, Mr. Edward Hyde came forth. He signified Dr. Jekyll's negative side. Mr. Hyde started to kill those people who did not offer assistance to Dr. Jekyll. It is astonishing that the same can happen to men as they go into transition in andropause period. Their attitudes are bound to change from gentle to fierce, from the loving sensitive man or husband to being uncaring and totally mean.

Counseling is very helpful for anyone with IMS to prevent from becoming a Mr. Hyde. No one wants to be like him for sure. Convenient steps like maintaining a healthy and nutritious diet and exercise are beneficial enough. While those with low levels of testosterone can be amended with testosterone replacement therapy. The entire condition is baffling enough for one to take, how much more to those people around. As always choose the whole-grain breads and cereals whenever you can. If that’s not an option your second best choice would be to get the enriched versions of refined breads.

Here’s a quick word of word of advice when you’re selecting muffins, quick breads, and breakfast pastries. That mouth watering Danish that you see in the cake section is probably overflowing with fats as well as sugar. This can also be true with many store-bought granolas and for the ridiculous amount of butter you might be tempted to smear on that healthy piece of whole-wheat bread.

Remember, breakfast is supposed to rejuvenate and give you loads of energy for your day. Consuming lots of fat and sugars will get your day off to a sluggish start.

Fruits and Vegetables

A piece of fresh fruit is the supreme choice from this food group. Raw fruit is actually better than juice because of the fiber that you’ll get. If you really can’t start your day without a glass of juice then it’ll be your second best choice. Dried fruits are great to add as toppings to hot or cold cereals, especially for women who are trying to boost their iron intake.

When the brilliant colors of autumn are gone and the weather starts to turn a bit nippy, we look ahead to the holiday season with wonderful images of the past dancing in our heads. Its a magical time when we are reunited with friends and family; a time when children are the happiest and all of us are at our very best. Needless to say delicious food will be the cornerstone of all this festivity.

Whether its Thanksgiving, Christmas or New Years, its truly a challenge to prepare food that is acid reflux friendly. During the holiday season, we are tempted to indulge in all those fatty, sweet and acidic foods that we enjoyed so much in childhood. We like to think of this as comfort food, however, acid indigestion is anything but comfort to those who suffer from it.

Complete abstinence from acidic, fatty and sweet food would defeat the purpose of celebrating the holidays. Dishes that do not bring back memories of childhood would do the same. Thankfully, there are simple ways to make seasonal dishes healthier without ruining all the fun.

If you cant resist that holiday drink or chocolate dessert then it would be wise to let moderation be your guide. Also, keep in mind that eating too much of even the most alkaline food can cause indigestion, as well.

I have created what I consider to be the perfect holiday menu. The dishes below retain the important flavors of the season, yet are healthy enough for most acid reflux sufferers to enjoy in comfort. The entire family will love this dinner. Its also quick and uncomplicated to prepare. This menu will feed six easily with leftover beef.

SOUP COURSE

Country Style Creamed Pumpkin Soup

1 pat of unsalted butter

cup natural chicken stock (Kitchen Basics, if possible)

1 Cup finely minced yellow onion

15 oz can pure pumpkin

Chicken stock (fill empty pumpkin can)

cup maple syrup

tsp. cinnamon

Freshly grated nutmeg

cup half & half

Crumbled nitrate free bacon, chopped chives and sour cream in squeeze bottle (optional)

In a 2 qt. soup pot heat butter and stock to the simmer.

Add minced onion and cook covered stirring occasionally until tender and translucent.

Blend in pumpkin and can of stock. Cook at the simmer for 10 minutes.

Blend in maple syrup and spices. Simmer another 5 minutes.

Take off heat and add half & half and mix well.

Note: This dish is best made the day before. Reheat in a double boiler.

Serve in a soup terrine and offer garnish on the side. Garnish with bacon for the non-gerd guests and chives for others. After plating the soup, make decorative zigzag patterns with sour cream from the squeeze bottle.

MAIN COURSE

Roasted Beef Tenderloin with Caramelized Root Vegetables

1 (4 to 5 pound) trimmed & tied tenderloin roast

Olive oil

Head of garlic, peeled and sliced

Salt and pepper

Fresh rosemary chopped

3 parsnips & 3 turnips peeled and cut into 1 pieces

pound peeled baby carrots

15 small pearl onions, peeled (make an X with knife at the root end)

1 pound new potatoes cut in half

Olive oil

Salt and pepper

Fresh chopped thyme

Parsley for garnish

Heat oven to 425 degrees F.

Place roast in roasting pan.

Massage thin coat of olive oil into roast.

Rub sliced garlic over roast and discard.

Season with salt & pepper to taste.

Insert meat thermometer into thickest area of roast.

Place vegetables in bowl & coat well with olive.

Season with salt & pepper to taste and mix well. Sprinkle with chopped thyme.

Empty vegetables into roasting pan around beef.

Roast for 40 50 minutes until the thermometer reads 140 degrees F.

Stir vegetables several times to keep moist.

When beef is done, remove to warm platter and tent with foil.

Let roast rest for 20 minutes.

If vegetables are not done, stir and continue to roast until tender and caramelized. Garnish with parsley.

Serve with horseradish sauce to those who are non-gerd and au jus from the cutting board, for those more sensitive.

Note: If using a convection roasting oven, adjust the cooking time accordingly.

SALAD/CHEESE COURSE

Belgium Endive, Toasted Walnuts & Blue Cheese with Walnut V inaigrette

6 heads Belgium endive leaves washed & dried

1 cup toasted walnuts (toast lightly in heavy pan and shake skins off)

pound crumbled good blue cheese (Gorgonzola, Stilton or Roquefort)

4 tbsps. Sherry vinegar

1 large shallot finely minced

cup walnut oil

cup olive oil

Salt & pepper to taste :



In a bowl, whisk vinegar & shallots together. Slowly drizzle oil whisking continually until dressing becomes thick & creamy. Season with salt and pepper.

Toss together in a salad bowl; endive, walnuts, blue cheese and dressing.

DESSERT COURSE

Rustic Apple Tart with Apricot glaze

5 Golden Delicious apples, cored, peeled, quartered & sliced inch thick (toss in bowl with a little lemon juice to retard discoloration)

cup sugar

1 tsp cinnamon

cup apricot jam

cup dark rum

1 sheet commercial puff pastry (Dufour if you can find it, or any brand in the super market freezer case)

Preheat oven to 375 degrees F.

Slice apples with a mandolin or by hand and place in bowl tossing with lemon juice.

Place sheet of pastry on floured surface and turn edges in to form a border.

Place pastry on parchment paper covered cookie sheet & adjust.

Arrange apple slices neatly overlapping in alternate rows.

Sprinkle with sugar and cinnamon.

Place on middle rack of hot oven.

Bake for 40 45 minutes, or until golden brown.

Microwave apricot jam & rum in plastic wrap covered bowl on high for 30 seconds. Whisk to blend.

Paint finished tart with apricot mixture with pastry brush while still warm.

Serve with ice cream or crme fraiche. Garnish with a sprig of mint or fresh raspberries.

Note: This wonderfully festive dessert can be prepared hours before dinner is served. If you should use the wrong apple and, or if your apple patterns go amiss, dont despair. You cant go wrong with this rustic dish. It will look appetizing anyway it turns out. Just say that you intended it to be that way!

What is the cause of female infertility?

Infertility is a situation faced by many couples worldwide when they are unable to conceive. According to a survey in America, around 10% of couples bear the brunt of infertility. Usually when a couple is not able to conceive, the female partner is generally blamed, but the reality is that both the partners are equally responsible.

The most common cause of female infertility is an ovulation disorder. Other causes of female infertility include blocked fallopian tubes, polycystic ovary syndrome (PCOS) and endometriosis. Repeated miscarriages may be caused by congenital anomalies (birth defects) involving the structure of the uterus and uterine fibroids.

Signs and symptoms

The main symptom of infertility is the inability for a couple to get pregnant. There may be no other obvious symptoms of infertility.

In some cases, an infertile woman may have abnormal menstrual periods. An infertile man may have some signs of hormonal problems such as changes in hair growth or sexual function.

Diagnosis methods

Investigating suspected infertility requires a number of tests for both the woman and her partner. Tests for the woman may include:

* Blood tests - to check for the presence of ovulation hormones. * Laparoscopy - a 'keyhole' surgical procedure in which an instrument is inserted though a small incision in the abdomen so that the reproductive organs can be examined. * Ultrasound tests - to check for the presence of fibroids.

Treatment

The treatment of infertility has made enormous progress in the last decade as a result of advances in assisted reproductive technology, or ART. This technology combines the use of fertility drugs - hormonal therapy - with artificial insemination using any of a group of techniques: intrauterine insemination (IUI), in vitro fertilization (IVF), gamete intrafallopian transfer (GIFT), zygote intrafallopian transfer (ZIFT), or oocyte (egg) donation.

Acupuncture may be helpful in the treatment of some cases of female infertility due to problems with ovarian function. In a preliminary trial, women who did not ovulate were treated with acupuncture 30 times over three months. Effectiveness was determined by a combination of measures indicating ovulation was returning to normal.

How can I help prevent infertility?

You may not be able to prevent infertility resulting from genetic problems or an illness. However, you can do the following to reduce the risk of infertility:

* Prevent sexually transmitted diseases by using latex or polyurethane condoms. Also, have just 1 sexual partner who is not sexually active with anyone else. * Limit the amount of alcohol you drink to no more than 1 or 2 drinks a week. * Avoid the use of street drugs (such as heroin) and overuse of prescription and nonprescription drugs. * Do not smoke. * Maintain good personal hygiene and health practices.

Infertility is a tough situation for women. Many of them conclude that they can't conceive, but that is not true. At times it takes long to conceive and even if then one is not able to, and then these medical aids are there to help one out of these difficult situations.

There are many definitions and symptoms commonly described as “scalp infections". There are in fact quite a few quite different reasons why your scalp may be sore, itchy, inflamed burning red or infected. If you are like many people with chronic scalp issues – it may be driving you completely up the wall and not knowing what it is or how to treat the problem effectively after many unsuccessful attempts to eliminate it can make it seem far worse - like you are never going to get to the bottom of the problem. Well that need no longer be so! It can be simple to diagnose and treat it now and for good.

So if you are dealing with any of these symptoms lets first identify the problem and then some of the underlying causes.

Firstly, if you want to get to the root of your problems, so to speak, you need to be able to identify what your problem actually is. Of course, many scalp infections & conditions look alike, so here is a brief description of some of the most common scalp conditions:

    Red burning itchy scalp can be characterized as any tingling,burning, prickly, and sensitive to the touch or a fiery hot sensation. Often associated with an allergy, sunburn, chemical burn (sensitization of the scalp),or fungal infection.

    Dandruff as most of us know is the process of continual shedding of skin cells on our scalps. Often the cause of dandruff is simply the result of toxins, pollutants and products that have built up on the scalp particularly products like silicone– a cheap commonly used artificial shine enhancer in conditioners sold in supermarkets.

    When the natural balance of our scalps is disturbed, this creates the perfect environment for the yeast fungus Melassezia Globbosa to move in and thrive. This is when the natural process of shedding our cells gets a little out of hand resulting in an unsightly condition known as “Seborrhoeic Dermatitis" (fancy name for dandruff) and we see the highly visible tell-tale signs of flaking and crusting.

    The accompanying itch and urge to scratch are often made worse by the multiplication of the fungal yeast pityrosporum-ovale (having one big party on your scalp). Another follow on effect of this is that the relentless scratching by the sufferer can cause small lesions which weep and ooze or become infected resulting in scalp infections.

    Lumps bumps and sores are also common when the scalp is highly irritated or sensitivity has been aggravated. Candida can be an underlying cause of yeast infections of the scalp.

    Ringworm of the scalp (tinea capitols) is a superficial fungal infection of the scalp. Scalp ringworm is caused by mold-like fungi called dermatophytes. Ringworm infection occurs when a particular type of fungus grows and multiplies anywhere on your skin, scalp, or nails. It is far more common in children and symptoms include red, itchy patches on the scalp, leaving bald areas. The skin might itch and be red and peel or be scaly, have swollen blisters or a rash (that can spread) and looks like black dots. The rash is highly contagious. It is normally treated with over the counter products containing miconazole, clotrimazole, or similar. Sometimes prescription anti fungal skin medications, such as ketoconazole are needed to clear it up. There are also products direct from nature’s factories that can aid the healing process and act as natural antibiotics, although it must be said that ringworm is an aggressive fungus which needs to be monitored closely and treated accordingly.

    Dry scalp can feel “tight", a sensation that is sometimes accompanied by flakiness. It is often the result of natural oils being stripped from our scalps by the frequent use of shampoos, hair dyes and or other hair products.

    Dermatitis of the scalp (Seborrheic dermatitis) is an inflammatory disorder affecting areas of the head and body where sebaceous glands are most prominent. It can vary from mild dandruff to dense flakey and greasy scale. Once again it is often an accumulation of toxins and products built up on our scalps that our body is trying to rid itself of.

    Eczema of the scalp similar in appearance to Seborrheic dermatitis but instead has the name Atopic dermatitis.

    Scalp Psoriasis commonly occurs on the back of the head however multiple areas of the scalp or the whole scalp may be affected. Scalp psoriasis is characterized by thick silvery white scales on patches of very red skin and can extend slightly beyond the hairline. Scalp psoriasis, despite being partially hidden by the hair is often a source of social embarrassment due to flaking of the scale and severe 'dandruff'. Scalp psoriasis may be extremely itchy or on occasion have no itch symptoms. It can also cause temporary baldness on the affected areas. It is a common, chronic, inflammatory skin disease and is associated with increased risk of melanoma, squamous cell carcinoma, and basal cell carcinoma.

    Folliculitis of the scalp is a superficial bacterial infection of the hair follicles. It is characterized by pustules around the hair follicles and symptoms include painful brushing of hair and tenderness when rubbing your scalp around the hair follicle sites. Treating with antibiotics for the particular bacteria is the usual course of action but can eventually cause resistance to the antibiotic used. Options include applying Evoclin Foam which is topical. Folliculitis is commonly caused by staph bacteria which also reside inside the nose and sinus cavity. Natural oils such as tea tree and lavender have good antibacterial properties and can also be used to treat this condition.

    If you have a bump or lump under the skin (commonly a lump behind the ears with no broken skin or rash developing, then this could be a cyst. Cysts are liquid filled pustules which can be painful when they become infected. If you have such symptoms I recommend seeing a doctor to be safe. Doctors can also treat cysts with antibiotics if they have become infected or incise them as a further but uncommon procedure if the antibiotics are not sufficient.

    Stress Related Itching Stress can contribute to or worsen scalp itching as it plays havoc with our immune system and hormones which have an effect on production of sebum from your sebaceous glands. Although it can contribute to the problem it is still somewhat of a myth. It may appear that you have a“stress rash", but it is more likely that the problem was already there in a less noticeable way, then aggravated by increased stress levels making it harder for your body to deal with the problem on its own and eliminate the problem. Many of the treatments below include stress relieving properties – like Lavender oil for example which calms the mind and soothes the body.

    Solutions – what YOU can do about it

    Going to your doctor may be the best way to get an accurate answer to diagnosing your problem, but I understand that many of you may have already done this, been prescribed some form of medication of cream (even “heavy duty" cortisone based ones where the cortisone potency may need to be increased after time).

    Often the problem either doesn’t go away from the start – or seems to subside but comes back with full a vengeance later leaving you wondering with more unanswered questions and a VERY unhappy scalp! Well the good news is if you don’t like using these expensive and often impotent products– or even want to help them along you can use natural remedies and essential oils known to be super effective anti-bacterial busters such as tea tree oil for instance to inhibit or stop the growth of the infection and assist healing/re balancing of the scalp.

    Something that is also important to understand in curing scalp conditions and scalp infections – a factor that is missed by so many doctors, and something not disclosed by companies who sell hair products and treatments is the number of people who are allergic or sensitive to commonly used ingredients in shampoos, dandruff treatments and other hair products. In fact MOST hair products (as many as 90%) contain at least one ingredient that is used in engine degreasers, another is a common ingredient in antifreeze.

    These ingredients are cheap to produce foaming agents that make shampoo foam up, and do a fantastic job of stripping dirt and oily build up from hair. Problem is that they sometimes do SUCH a good job that they remove all of the natural oils and sebum from the scalp thus removing the scalps natural against bacteria etc. Another little known fact is that the most common ingredient (Sodium laureth sulphate) is used to induce skin irritation in test patients by drug companies to test the effectiveness of their itch relief skin creams. When first produced and for year’s afterwards manufacturers did not know that skin was not a barrier but an absorber of ingredients & that they caused skin irritation in so many people.

    So sometimes the beginning of a scalp condition can have originated from something as simple as an imbalance to the skins (scalps) sebum or an allergy to an ingredient that can easily be rectified.

    Like your immune system, if your skin has an opportunity to rebalance itself and not be subjected to “incoming foreign matter" and the daily stripping of its natural oils that further aggravates the problem you can allow your body to re balance and heal itself. Obviously buying products that do not strip your hair and scalp is an obvious step in the right direction.

    Fortunately there are a lot of manufacturers such as Aveda and Akin (Purist Company) who know what consumers are suffering from these issues so have addressed this problem. They have applied this knowledge to produce some amazing products with all of the benefits of natural ingredients, with none of the side effects at a price we can afford (Akin especially). It need not be difficult, nor expensive to make these choices and a move to healthy hair and scalp long term.

    So yes you can eliminate harsh shampoos containing sodium sulphates and go for natural Sulphate and paraben free shampoos that contain ingredients such as macadamia, wheat germ or jojoba oils. These oils mimic our natural sebum to some extent to help to re moisturize and nourish our scalp.

    Then there are lavender, rosemary and tea tree oils which contain antibacterial, healing properties. Neem oil great for its antiquing and stops itching. You can apply these topically to your skin for fast relief, to assist healing as well as looking out for shampoos with these natural ingredients in them.

    Lavender calms the skin (as does chamomile tea cooled and used as hair rinse).
    As well as buying quality shampoos containing these ingredients, you can
    create your own combinations of remedy or remedies to soothe your scalp.
    You can also help it along by using natural remedies to treat your scalp such as this scalp treatment containing natural oils to assist healing, detoxify and stimulate hair growth: (Add to

    I recently attended a meeting of physicians, surgeons, research scientists and concerned individuals for the Pacific Prostate Cancer Conference. Representatives of Oregon Health Sciences University spoke about their state-of-the-art prostate cancer research department. Every man reading this article can benefit from the advances made in prevention, detection and treatment of this disease.

    Let’s begin with a brief review of our anatomy. The prostate gland is located around the bottom portion of a man's bladder and is positioned behind the pubic bone and in front of the rectum. The prostate's primary function is to produce most of the fluids in semen. Tiny ducts within the prostate convey this fluid to the urethra, the channel that drains urine from the bladder. This fluid is then released through the penis during ejaculation.

    Prostate cancer is the most common cancer in American men. It's estimated that by age 50, up to 1 in 4 men have some cancerous cells in the prostate gland. By age 80, the ratio increases to 1 in 2. So, your risk of prostate cancer increases as you age. The average age at diagnosis is 72.

    Prostate cancer also is the second leading cause of cancer deaths in American men. Yet unlike other cancers, you're more likely to die with prostate cancer than you are to die from it. On average, an American man has about a 30 percent risk of having prostate cancer in his lifetime, but only about a 3 percent risk of dying of the disease.

    The risk of prostate cancer is dramatically higher among blacks, intermediate among whites, and lowest among native Japanese. However, this increase in risk may be due to factors other than those associated with race. Studies have shown a link between levels of testosterone and prostate cancer risk, with black men having the highest levels.

    Symptoms of prostate cancer include frequent urination, inability to urinate, blood in urine or semen, or painful ejaculation. Frequent low back pain, burning urination or a weak urine flow are also very common.

    There is another condition of the prostate known as Benign Prostatic Hyperplasia or BPH. This is basically an enlarged prostate with no cancerous lesions. Symptoms for this condition are similar in many respects to prostate cancer. For example, Prostate-Specific Antigen or PSA levels can become elevated in both BPH and prostatic cancer.

    Chemotherapy, radiation therapy and surgery are the three most common approaches to treatment of prostate cancer. The selection of which method is used depends largely upon the extent of the disease. With the surgical removal of the prostate, there is an increased risk of osteoporosis similar to that of menopause in women. This problem is most often handled by using hormone replacement therapy postoperatively. Calcium and Vitamin D supplements have also been shown to be helpful in reducing this type of bone loss.

    Natural measures for the prevention of prostate cancer include chemoprevention, changes in diet and lifestyle, and hormonal prevention. Chemoprevention is the use of specific natural or man-made drugs, vitamins, or other agents in order to reverse, suppress, or prevent cancer growth. Several agents, including difluoromethylornithine (DFMO), isoflavonoids, selenium, vitamins D and E, and lycopene have shown potential benefit according to studies. Further tests are needed to confirm this.

    The herb, saw palmetto, was once thought to be the best natural supplement for prevention of prostate cancer. Recent studies lead researchers to believe saw palmetto to be much less powerful in its preventive effect.

    A diet high in fat, especially animal fat, may be associated with an increased risk of prostate cancer. So, reducing red meat, cheese and other dairy products, salad dressings, and gravies is a good step. Increasing the dietary intake of fruits and vegetables has been associated with a reduced risk of prostate cancer in some studies. Adding soy products such as tofu and soy milk can be helpful in regulating hormonal balance. Exercise, even just plain old walking, has been shown to reduce the incidence of prostate and of practically all other cancers.

    Since prostate cancer has a link to testosterone levels, studies are underway to discover the role of certain drugs, such as finasteride, that reduce the amount of male hormone as preventive agents for prostate cancer.

We hypothesize that factors exist which can be modified by clinicians to enhance outcome. A retrospective cohort study design was used. Cohort members had one implant placed. Risk factors were classified as demographic, health status, implant-, anatomic-, or prosthetic-specific, and reconstructive variables. The outcome variable was implant failure. The cohort was composed of 677 patients who had 2349 implants placed. Based on the adjusted multivariate model, factors associated with implant failure were tobacco use, implant length, staging, well size, and immediate implants (p 0.05). In the setting of correlated survival observations, we recommend adjusting for the correlation of the observations to provide statistically valid and efficient results. Three of the identified factors--tobacco use, immediate implants, and implant staging--potentially may be modified to enhance implant survival.

    Surgical complications - in addition to the usual risks of a surgical procedure, implantation can cause infection of the gums or bone and can damage the roots of neighboring teeth, the nerves leading to the lip or the sinus cavity. Skilled dentists rarely encounter these complications, but you face significant risks in the hands of an inexperienced practitioner.

    For some people, particularly persons with loose or poor fitting dentures due to flat ridges, or persons with multiple missing teeth who need support for crowns and bridges, implants may be considered a more appropriate alternative to fix the appearance of missing teeth than dentures.     Anti-obesity, health advocates are at it again. They have successfully won a battle on having sodas banned in elementary schools by the 2008-2009 school year. They are furthering their cause by trying to also ban sports drinks in schools such as 'Gatorade' and 'Powerade' and flavored waters. They are claiming that sports drinks contain as much as two-thirds of the sugars in sodas, and more than three times the sodium.

    Senator Tom Harkin (D-Iowa) has apparently agreed with this cause, enough, by introducing a bill in congress that would allow the government to decide what is best for the nation's children, in setting nutritional standards for all foods, drinks, and flavored vitamin waters in schools. The hotly debated topic ensues in congress between nutritional health care advocates and the trade group that represents the bottlers of these drinks. Are they really 'safer alternatives' or just 'junk food'? It certainly is garnering some attention in congress, and for other interested bystanders as well.

    The trade group representing the manufacturers of these drinks, of course, are adamantly opposing this bill. They proclaim these drinks are lower in sugar and calories than sodas, and are appropriate, even essential, to young high school athletes.

    But, there is more evidence coming from the University of California at Berkeley, that adds fuel to the health care advocates cause, that warns about the possible weight gain associated with consuming to many of these drinks. They conclude that students who drink one 20- ounce of the drinks in question per day, for a year, without sufficient physical activity may experience a weight gain of up to 13 pounds.

    Nutritionists, and health care advocates are not at all surprised with these findings. They say that looking at the ingredient label pretty much says it all. Read what the first three to four ingredients are in a recipe, they are listed in the order of their prevalence in the mixture. Most sports drinks and flavored waters consist mostly of water, many different types of sugar, acid flavor enhancers, and sodium.

    Bucket loads of money is being spent by beverage manufacturers, on brainwashing advertisements, to get consumers to believe that these drinks are healthy, and essential. They claim they are needed to replenish those lost electrolytes, due to physical exertion. But, there is simply no real scientific evidence to support this theory. So, you can then begin to understand why the makers of these drinks do not want any official congressional sanctioning, of the notion, that sports drinks are linked to obesity.

People must have a bath daily, preferable in hot water as this also increases the circulation of blood within the body. After the bath, the body must be wiped with a rough towel to remove the dead bacteria and dead cells off the body.

    Wearing clean clothes after a bath every morning is ideal. A talcum powder which has antiperspirant agents in it must be used before setting out of the house every morning. Also clean socks and underwear are important.

    Most of the body odor problems arise from the armpits. Shaving the hair from this region reduces the problem of body odor to a great extent.

    (4) Home Medications

    ® Some herbal teas can eliminate body odor. Sage tea, when had daily once, can eliminate the bacteria that are responsible for body odor.

    ® Sage leaves are also helpful in the removal of unwanted body smells. Sage leaves can be mixed with tomato juice and apply this on the regions which are he sources of the body odor. After about thirty minutes, have your bath and wash off the paste. You will remain body odor free for the whole day.

    ® An unconventional method to reduce body odor is to use vinegar made from apple cider in place of deodorants. This is used in the armpits. Though the method is less commonly used, results are obtained. Vinegar contains acetic acid which is an antibacterial. Even normal white vinegar when applied to the areas of profuse perspiration will reduce most of the bacteria living there and eliminate the body odor.

    ® Turnip is also known to help. The juice of a grated turnip when applied to the places where sweat is more will keep that area free of sweat and perspiration for a long time. Regular use of this method will eliminate the problem of body odor completely.

    6.) Remember to floss between all of your teeth, even the hard to reach ones in the back of your mouth. Also floss the back of the last teeth.

    7.) Remember this is a gentle motion. Do not use a frontward, backward sawing motion. That can cause injury and prove to be painful.

Ibuprofen, a NSAID is another alternative. You should not use aspirin in a child under the age of 16, but it can be very helpful for the older age groups.

    In many countries, the standard treatment for otitis media has been a course of antibiotics. There is currently some debate as to whether this should indeed be the treatment. There are points for and against their use.

    Boerhaave's Syndrome - Spontaneous Rupture of the Esophagus

    Case Presentation:
    An 80 year-old female with a past medical history of hypertension, renal cell carcinoma, and status-post nephrectomy, experienced an episode of choking, coughing and vomiting when she had difficulty swallowing her Centrum Silver tablet. Soon after vomiting, the patient developed severe chest pain with radiation to her back. The pain was significantly worse with deep inspiration.

    Introduction:
    Boerhaave's Syndrome is a spontaneous rupture of the esophagus, classically described as postemetic, but may also occur after lifting a heavy weight, with severe asthma, or prolonged coughing. The sudden rise in esophageal intraluminar pressure causes an esophageal tear which most commonly occurs in the lower esophagus above the left diaphragm.

    Boerhaave's Syndrome was first described in 1724 by Hermann Boerhaave, a Dutch physician. His patient was a 50 year-old admiral (Baron John van Waasenaer) who developed a sudden excruciating chest pain while straining to vomit. The patient developed shock and died 18 hours later. An autopsy demonstrated rupture of the distal esophagus into the left chest.

    Clinical Presentation:
    Classically, the patient is a middle-aged, white male with a history of overindulgence in food or drink. The patient develops vomiting, lower chest pain, and mediastinal or subcutaneous emphysema (Meckler’s triad – vomiting, pain and subcutaneous emphysema).

    The physical exam typically reveals a critically ill patient, usually sitting up in bed with a forward-crouching position. Subcutaneous emphysema is frequently seen. Hamman’s sign is seen in up to 20 percent of Boerhaave's patients. Hamman’s sign is a crunching, rasping sound, synchronous with the heartbeat, heard over the precordium, and is often indicative of spontaneous mediastinal emphysema. Patients also present with varying degrees of epigastric tenderness, sometimes imitating an intra-abdominal catastrophe. When the rupture is confined to the mediastinum, the patient may not look particularly sick and vital signs may be deceptively normal.

    Differential Diagnosis:
    The differential diagnosis of Boerhaave's Syndrome is extensive and should include the following:

    • Perforated or Bleeding Ulcer
    • Acute Pancreatitis
    • Myocardial Infarction
    • Pulmonary Embolus
    • Dissecting Aneurysm
    • Spontaneous Pnuemothorax
    • Mallory-Weiss Tear
    • Acute Cholecystitis
    Diagnostic Evaluation:
    In addition to the patient history, radiography remains the cornerstone of the diagnostic evaluation for Boerhaave's Syndrome. Plain chest radiographs may show mediastinal or free peritoneal air (most common finding on initial films), a widened mediastinum, hyrdrothorax, hydropneumothorax, or mediastinal emphysema. Ten to fifteen percent of all patients presenting with Boerhaave's Syndrome may have a normal plain chest radiograph.

    A swallow contrast radiographic study remains the diagnostic gold-standard. Either a thoracic CT scan or an esophagram is required to locate the exact site of perforation, and helps to determine the best surgical approach. A water-soluble contrast agent such as gastrografin is utilized. Most recommend avoidance of barium since its penetration into the thorcacic cavity can induce an inflammatory reaction leading to granuloma formation.

    Management:
    The initial emergency department management of Boerhaave's Syndrome includes strict NPO (nothing per mouth), broad spectrum antibiotics, fluid resuscitation, and continuous nasal gastric suctioning. A cardiothoracic surgeon should be consulted urgently, and if cardiovascular services are not available at your facility, the patient should be transferred to an appropriate facility.

    Patient’s are often placed on total parenteral nutrition, and early surgical repair remains the standard of care. Complications of Boerhaave's Syndrome include persistent esophageal leak, mediastinitis, polymicrobial sepsis, pneumonia and empyema.

    Despite optimal management, the mortality of patient’s with Boerhaave's Syndrome remains high. Mortality rates have been quoted as high as 72 percent and are most likely attributable to difficulty in making the diagnosis. In contrast to spontaneous rupture of the esophagus, iatrogenic esophageal rupture carries a mortality rate of only 20 percent, and traumatic perforation has a mortality of only 7 percent.

    Case Conclusion:
    The patient was treated in the emergency department with Aspirin, Morphine, Reglan, one liter of normal saline, Zosyn 3.375 grams intravenous, and a nasogastric tube was placed. A thoracic CT scan with oral administration of 20 ml Redicat demonstrated bilateral pleural effusions, with a tract of contrast and air noted within the anterior wall of the esophagus. A semi erect single contrast esophagram with thin barrium solution demonstrated an esophageal tear adjacent to a short esophageal stricture in the mid to distal 1/3 of the esophagus. The gastro-intestinal and cardio-thoracic surgery services were consulted and the patient was transferred to the ICU .

    Canine distemper is a very life-threatening disease, which is triggered by a highly contagious virus that attacks the gastrointestinal, respiratory and the nervous systems of dogs. A veterinarian can best determine when is the proper time to begin your animal's vaccination by basing their decision on the dog general health and his own experience as a vet.

    It is imperative to closely observe any hints of ill health as well as regular care to maintain and assure a general good condition and health, for your animal. If your pet shows any of the signs below, experts encourage you to consult with your veterinarian immediately:

    3500 Calories: A Pound of Fat or Six Pounds Of Muscle?

    Most fitness conscious people know that there are 3,500 calories in a pound of fat, so if you create a deficit of 3500 calories in a week, you lose a pound of weight. If you create a deficit of 7000 calories in a week, you lose two pounds a week, and so on. Right? Well, not so fast...

    Dr. Kevin Hall, an investigator at the National Institute of Health has done some interesting research about the mechanisms regulating human body weight. He recently published a new paper in the International Journal of Obesity that throws a wrench in works of the "3500 calories to lose a pound" idea...

    Some of the equations in his paper made my head hurt, but despite the complex math he used to come to his conclusions, his article clearly prompts the question, "3500 calories to lose a pound of WHAT?" His paper also contained a lot of simple and practical tips you can use to properly balance your caloric intake with output, fine tune your calorie deficit and help you retain more muscle when you diet.

    Below, I've distilled some of the information into a simple bullet-point summary that any non-scientist can understand. Then I wrap up with my interpretation of how you can apply this in your own fat loss program:

    Calculating the calories required to lose a pound and fine-tuning your caloric deficit

    * 3500 calories to lose a pound has always been the rule of thumb. However, this 3500 calories figure goes back to research which assumed that all the weight lost would be adipose tissue (which would be ideal, of course).

    * But as we all know (unfortunately), lean body mass is lost along with body fat, which would indicate that the 3500 calorie figure could be an oversimplification.

    * The amount of lean body mass lost is based on initial body fat level and size of the calorie deficit

    * Lean people tend to lose more lean body mass and retain more fat.

    * Fat people tend to lose more body fat and retain more lean tissue (revealing why obese people can tolerate aggressive low calorie diets better than already lean people)

    * Very aggressive low calorie diets tend to erode lean body mass to a greater degree than more conservative diets.

    * whether the weight loss is lean or fat gives you the real answer of what is the required energy deficit per unit of weight loss

    * The metabolizable energy in fat is different than the metabolizable energy in muscle tissue. A pound of muscle is not 3500 calories. A pound of muscle yields about 600 calories. • If you lose lean body mass, then you lose more weight than if you lose fat.

    * If you create a 3500 calorie deficit in one week and you lose 100% body fat, you will lose one pound. •

    * But if you create a 3500 calorie weekly deficit and as a result of that deficit, lose 100% muscle, you would lose almost 6 pounds of body weight! (of course, if you manage to lose 100% muscle, you will be forced to wear the Dieter's Dunce cap)

    * If you have a high initial body fat percentage, then you are going to lose more fat relative to lean, so you may need a larger deficit to lose the same amount of weight as compared to a lean person

    * Creating a calorie deficit once at the beginning of a diet and maintaining that same caloric intake for the duration of the diet after major weight loss fails to account for how your body decreases energy expenditure with reduced body weight

    * Weight loss typically slows down over time for a prescribed constant diet (the "plateau"). This is either due to the decreased metabolism mentioned above, or a relaxing of the diet compliance, or both (most people just can't hack aggressive calorie reductions for long)

    * Progressive resistance training and or high protein diets can modify the proportion of weight lost from body fat versus lean tissue (which is why weight training and sufficient protein while on calorie restricted diets are absolute musts!)

    So, based on this info, should you throw out the old calorie formulas?

    Well, not necessarily. You can still use the standard calorie formulas to figure out how much you should eat, and you can use a 500-1000 calorie per day deficit (below maintenance) as a generic guideline to figure where to set your calories to lose one or two pounds per week respectively (at least that works "on paper" anyway).

    Even better however, you could use this info to fine tune your caloric deficit using a percentage method and also base your deficit on your starting body fat level, to get a much more personalized and effective approach:

    15-20% below maintenance calories = conservative deficit

    20-25% below maintenance calories = moderate deficit

    25-30% below maintenance calories = aggressive deficit

    31-40% below maintenance calories = very aggressive deficit (risky)

    50%+ below maintenance calories = semi starvation/starvation (potentially dangerous and unhealthy)

    (Note: According to exercise physiologists Katch & Mcardle, the average female between the ages of 23 and 50 has a maintenance level of about 2000-2100 calories per day and the average male about 2700-2900 calories per day)

    Usually, we would suggest starting with a conservative deficit of around 15-20% below maintenance. Based on this research, however, we see that there can be a big difference between lean and overweight people in how many calories they can or should cut.

    If you have very high body fat to begin with, the typical rule of thumb on calorie deficits may underestimate the deficit required to lose a pound. It may also be too conservative, and you can probably use a more aggressive deficit safely without as much worry about muscle loss or metabolic slowdown.

    If you are extremely lean, like a bodybuilder trying to get ready for competition, you would want to be very cautious about using aggressive calorie deficits. You'd be better off keeping the deficit conservative and starting your diet/cutting phase earlier to allow for a slow, but safe rate of fat loss, with maximum retention of muscle tissue.

    The bottom line is that it's not quite so simple as 3,500 calories being the deficit to lose a pound. Like lots of other things in nutrition that vary from person to person, the ideal amount of calories to cut "depends"...

    There are ways to cope with your sweet tooth in your clean eating plan.

    Sugar: This is all natural. Some nutritionists (and your LIVE personal trainer) are of the opinion that if you are going to have sweetener of any kind, have the real thing! They feel that while it impacts your blood sugar the most, this is the safest choice as you can get it in the natural form. Many feel that knowing you are using the real thing will help you curb the quantity you chose to use.

    Stevia: Also a naturally occurring option. More and more it can be found in mainstream grocery stores in the baking isle right next to Splenda! Give it a chance. Some people really love it and find it to be an excellent alternative to other sweeteners. You can even buy it in convenient packets to carry with you!

    Agave Nectar: This can usually be found in the section with honey of a natural foods grocery store. It is a thick liquid similar to honey and can easily be added to many foods where you might normally use sugar to sweeten.

    Aspartame (Nutrasweet... the blue stuff): This is a chemical compound touted as being 180 times sweeter than regular sugar. But don't kid yourself! It has calories!!! 4 calories per gram to be exact. Phenylketonuria is also of concern. This is a genetic disorder in which the body cannot process some of the enzymes in the aspartame. Don't start running to the doctor just yet. Chances are, if you have it, you already know it. All products containing aspartame must also carry the warning "Phenylketonurics: Contains Phenylalanine" on the label so that they are easily identified.

    Sucralose (Splenda): This sugar derivative is 320 - 1000 times sweeter than sugar itself. It is manufactured through selective chloronation of sucrose. Since it is stable under heat, it can even be used in baking! Note that although the nutritional label on Splenda's packaging states that a single serving (1 teaspoon = 5 g) contains zero calories, Splenda actually contains two Calories per teaspoon.

    Saccharin (Sweet-N-Low... the pink stuff): This sugar substitute is 300 times sweeter that sugar. However, some don't care for it because of an "aftertaste". Some soft drinks are sweetened with saccharin. Some studies have linked it to the development of cancer. Therefore, saccharin products are required to carry the warning "Use of these products may be hazardous to your health. This product contains saccharin which has been determined to cause cancer in laboratory animals."

    Fruit: If fruit seems to satisfy your sweet tooth, try incorporating lower glycemic choices into your diet! Strawberries are my personal favorite. Other choices might include, blue berries, razberries, or plums. Apples, oranges, and grapefruit are also excellent options since they are accompanied by high fiber content. This slows the glycemic reaction to the fructose in the body.

    In the end, none of these options are without impact to your clean eating diet. However, they are ways to address and manage your nutrition plan in a way that sets you up for success. Ultimately, only you can decide what is right for you and your body. This is just a short list of some options to help you make an educated decision.

Congestive heart failure is a condition that can occur as a result of any structural or functional cardiac disorder which impairs the ability of the heart to fill with and pump a sufficient amount of blood throughout the body. Since not all patients experience volume overload at the initial evaluation, this disease is most commonly referred to as heart failure, rather than congestive heart failure. This disease is often under-diagnosed because of a universally agreed upon definition does not exist and there are difficulties with diagnosis, especially in those with a mild condition. Even with the best care and therapy, heart failure has an associated mortality rate of ten percent and is the leading cause of hospitalization in people above the age of sixty-five.

    Many studies have been done and presented at the Second Annual Academic Surgical Congress, which is sponsored by the Association for Academic Surgery and Society of University Surgeons. Heart attacks often lead to the progressive onset of congestive heart failure. After a heart attack, the portion of the heart that hasn't been damaged must work harder to compensate for the functional loss of the tissue that is damaged. The heart will eventually begin to fail, causing the chambers of the heart to enlarge and waken, which in turn causes the heart to lose its ability to pump blood effectively.

    A study, which was conducted by the University of Minnesota, led by Dr. John Foker and his team, determined that administering D-Ribose will significantly improve heart function after a heart attack. The study also showed that the progression of heart failure after a heart attack begins with the fall of energy levels. D-Ribose is very unique, it serves as a foundation for many important compounds such as DNA, RNA, and ATP (the energy of cells). ATP is very important to health and for maintaining normal energy-dependent body functions. Hearts that are stressed, muscles, and other tissue lack the metabolic machinery that is needed to quickly make D-Ribose and overcome progressive energy depletion.     Do You Recommend Exercise For Your Patients With Osteoarthritis Of The Knee?

    Osteoarthritis (OA) is the most common form of arthritis and affects roughly 30-40 million Americans. The condition is due to a metabolic disorder of cartilage, the gristle that caps the ends of long bones. Cartilage functions to cushion the joint against impact and also to allow gliding of the joint with movement.

    Cartilage consists of cells called chondrocytes that sit inside a matrix- much like grapes inside of gelatin. Normally, chondrocytes produce substances that build up and nourish the matrix. When osteoarthritis develops, the chondrocytes begin to function abnormally and no longer elaborate normal matrix.

    Instead, chondrocytes begin to produce destructive enzymes that weaken the matrix. Small cracks in cartilage develop. At the same time, the lining of the joint (synovial capsule) is stimulated to produce inflammatory enzymes that further weaken cartilage. The end result is loss of cartilage. As cartilage wears away, the joint begins to become mechanically unbalanced leading to more damage.

    Conventional treatment for osteoarthritis of the knee consists of patient education, maintenance of ideal weight, supplements such as good quality forms of glucosamine/chondroitin, bracing, lateral wedge insoles, non-steroidal anti-inflammatory drugs, injections of glucocorticoids (steroids), injections of viscosupplements, and sometimes surgery. (All of these have been described in previous articles by me).

    One often neglected but critical component of treatment is exercise. A recent review discussed the importance of exercise and reached some interesting conclusions. (Bennell K, Hinman R. Current Opin Rheumatol. 2005; 17: 634-40)

    Benefits of exercise appear to be additive when exercise is delivered with other interventions such as weight loss.

    Types of exercise include strengthening, stretching, and non impact aerobic exercise, all of which are important and which work synergistically.

    It appears that supervised exercise sessions are superior to home exercises for pain reduction. The challenge is to convince patient so the importance of exercise. One interesting question is whether exercise can prevent disease or slow its progression.

    Also it is apparent that certain patient subgroups that may respond differently to treatment. It has been substantiated that one major risk factor for the development and progression of knee OA is weak quadriceps muscles. This is especially true for women. As a result, if strength training is administered to this high risk group, it may be able to prevent knee osteoarthritis.

    According to the authors, novel exercise programs that strengthen hip muscles or alter impairments in knee neuromuscular control may also influence disease progression.

    They conclude that "Future studies must identify cost-effective exercise modes, strategies to maximize exercise compliance and optimal treatment combinations. The role of muscle strength and altered neuromuscular control in the prevention and development of osteoarthritis must be evaluated with the view to devising and testing novel exercise interventions."

    As a rheumatologist who sees many many patients with OA of the knee, I encourage a comprehensive exercise program along with many of the other treatments described above. No one treatment will work for a patient. However, the right combination of treatments will work for almost all patients. Exercise is an oft-neglected but important component of a comprehensive knee care program with OA.

    Stress Causes Early Menopause

    Women who are under constant pressure to do better can get menopause early.

    This was revealed by French researchers who said that stressful working conditions may trigger the onset of the menopause.

    In their report published in the American Journal of Epidemiology, the researchers studied 1,500 postmenopausal women. Nearly nine percent said they had stressful jobs.

    "High-strain jobs were categorized as those which demanded at least one of the following: the need to rush, perform several tasks at once or frequent interruptions when working. However, the impact of these conditions was minimized when the woman in question had a high degree of control over the work she did,” said BBC News.

    Members of this group were found to have menopause a year earlier than the other subjects. Dr. Bernard Cassou, who led the study, believes that stress affects the hormone balance and causes the aging of the ovaries.

    But a British fertility expert expressed doubts on the relationship between stress and early menopause.

    "People who want to find links do but there's a vast body of research which shows there is no evidence of a connection between stress and the menopause," said Professor John Studd, chair of the British Menopause Society. Experts, however, agree that stress can make the symptoms of menopause worse.

    Menopause generally takes place between the ages of 45 and 55 but it can start as early as 35 or as late as 60. Symptoms include hot flashes, insomnia, headaches, skin wrinkling, and emotional changes like forgetfulness, anxiety, irritability, and depression.

    “Menopause is not a disorder. Most women do not need treatment for it. However, if symptoms are severe, medicines may help. After menopause, women are more vulnerable to bone loss and heart disease. Exercise and eating right can help keep women healthy as they age,” said Medline Plus.
